Até 2011, os únicos tratamentos disponíveis para o melanoma metastático eram a quimioterapia e a imunoterapia com interleucina-2. O conhecimento mais aprofundado sobre a biologia molecular do melanoma e a identificação de mutações BRAF, que são as mais frequentemente encontradas, permitiram encontrar novos alvos terapêuticos que vieram alterar o prognóstico destes doentes. Atualmente, os tratamentos disponíveis para o melanoma metastático com mutação BRAF são a imunoterapia com inibidores do checkpoint imunológico (anti-PD-1 e anti-CTLA-4) e a terapêutica dirigida com inibidores BRAF e inibidores MEK. No entanto, permanece ainda controverso qual a terapêutica de primeira linha a instituir nestes doentes.Materiais e Métodos: Para a elaboração desta revisão da literatura foi consultada a base de dados PubMed. Incluíram-se, ainda, alguns artigos importantes complementares, nomeadamente relacionados com o estadiamento do melanoma e as posologias dos fármacos aprovados. Após a análise criteriosa de 73 artigos, realizou-se o presente trabalho.Resultados: As terapêuticas que foram aprovadas e estão atualmente disponíveis para o melanoma metastático BRAF mutado são a imunoterapia e a terapêutica dirigida, que vierem revolucionar o prognóstico destes doentes. Quanto à imunoterapia, o nivolumab e o pembrolizumab (ambos anti-PD-1) em monoterapia ou a combinação de nivolumab e ipilimumab (anti-CTLA-4) são tratamentos possíveis e demonstraram, com base em diversos ensaios clínicos, respostas duráveis apesar de terem um início de ação mais lento. Relativamente à terapêutica dirigida, são opções de tratamento, em monoterapia, o vemurafenib, o dabrafenib (ambos inibidores BRAF) e o trametinib (inibidor MEK) e, em associação, encorafenib (inibidor BRAF) com binimetinib (inibidor MEK), cobimetinib (inibidor MEK) com vemurafenib, dabrafenib com trametinib. Contrariamente à imunoterapia, a terapêutica dirigida apresenta rápido controlo tumoral. No entanto, o desenvolvimento de resistências continua a ser um grande desafio para este tipo de tratamento. Conclusão: Diversos estudos estão em andamento, sobretudo combinações de imunoterapia e terapêutica dirigida e sequenciamento de ambas, com o objetivo de determinar com mais clareza o melhor tratamento a ser instituído nos doentes com melanoma metastático BRAF mutado. Associadamente, é necessário ter-se em conta as características individuais dos doentes, nomeadamente a presença ou não de metástases cerebrais, nível de LDH e status funcional, para permitir uma terapêutica personalizada que melhore o seu prognóstico.Introduction: Metastatic melanoma has been associated with a poor prognosis, with overall survival rates at 5 years of 10%. Until 2011, the only treatments available for metastatic melanoma were chemotherapy and immunotherapy with interleukin-2. The more in-depth knowledge about the molecular biology of melanoma and the identification of BRAF mutations, which are the most frequently found, allowed us to find new therapeutic targets that came to modify the prognosis of these patients. Currently, the treatments available for metastatic melanoma with BRAF mutation are immunotherapy with immunological checkpoint inhibitors (anti-PD-1 to anti-CTLA-4) and targeted therapy with BRAF inhibitors and MEK inhibitors. However, the first-line therapy to be instituted in these patients remains unknown.Materials and Methods: To prepare this literature review, the PubMed database was consulted. Some important articles were also included to complement, namely related to the staging of melanoma and the dosages of approved drugs. After a careful analysis of 73 articles, this study was carried out.Results: The therapies that have been approved and are currently available for mutated BRAF metastatic melanoma are immunotherapy and targeted therapy. As for immunotherapy, nivolumab and pembrolizumab (both anti-PD-1) alone or the combination of nivolumab and ipilimumab (anti-CTLA-4) are possible treatments and have demonstrated, based on several clinical trials, durable responses despite have a slower start of action. Regarding targeted therapy, treatment options in monotherapy are vemurafenib, dabrafenib (both BRAF inhibitors) and trametinib (MEK inhibitor) and in combination encorafenib (BRAF inhibitor) with binimetinib (MEK inhibitor), cobimetinib (MEK inhibitor) with vemurafenib, dabrafenib with trametinib. Contrary to immunotherapy, targeted therapy has rapid tumor control. However, the development of resistance remains a major challenge for this type of treatment.Conclusion: Several studies are underway, especially combinations of immunotherapy and targeted therapy and sequencing of both, with the aim of determining more clearly the best treatment to be instituted in patients with mutated BRAF metastatic melanoma.
