Until 2011, chemotherapy, high-dose IL-2, and interferons, were approved as single therapies for the treatment of advanced melanoma, did not offer great benefits in survival and were associated with high toxicity. New therapies are being developed to improve prognosis and toxicity, such as immunotherapy (that inhibits immune checkpoints and stimulates the immune response against the tumor), and targeted therapy that inhibits the MAPK pathway that is overexpressed in melanoma. A literature review was carried out in order to evaluate the effectiveness of each therapy, compare them in order to try to understand the best approach for each patient.Immunotherapy has been the first-line choice; the ipilimumab-nivolumab combination presents the best results in survival. However,it is associated with non-reversible adverse immune effects. Therefore, it is recommended in patients without comorbidities and with advanced and aggressive tumors. Monotherapy with anti-PD-1 is also an alternative, as it offers survival advantages and less toxicity. For patients with melanoma BRAF mutations, present in 40-60% of patients, targeted therapy has shown high response rates and rapid response induction, although associated with the development of skin tumors such as SCC. Targeted combination therapy is preferable to monotherapy, with the encorafenib-binimetinib combination having the least toxicity. Despite these great advances, some patients end up developing secondary resistance to these treatments.O melanoma é o cancro cutâneo com maior mortalidade e em estadios avançados apresenta mau prognóstico. Até 2011 as únicas terapêuticas aprovadas no tratamento do melanoma avançado eram a quimioterapia, IL-2 em alta dose e interferões, que não ofereciam grandes benefícios na sobrevivência e associavam-se a uma alta toxicidade. Novas terapias têm vindo a ser desenvolvidas para melhorar o prognóstico e diminuir a toxicidade, como a imunoterapia (que inibe checkpoints imunes e estimula a resposta imune contra o tumor), e a terapia-alvo, que inibe a via MAPK sobre-expressa no melanoma. Foi realizada uma revisão de literatura com o objetivo de estudar a eficácia de cada terapêutica, comparar as diferentes terapêuticas entre si, de modo a tentar perceber qual a melhor abordagem para cada paciente.A imunoterapia tem sido a escolha de primeira-linha, apresentando a combinação ipilimumab-nivolumab os melhores resultados na sobrevivência, associando-se, no entanto, a efeitos adversos imunes não reversíveis. É recomendada em doentes sem comorbilidades e com tumores avançados e agressivos. A monoterapia com anti-PD-1 é também uma alternativa, já que oferece vantagens na sobrevivência e menor toxicidade. Em caso de melanoma com a mutação BRAF, presente em 40-60% dos doentes a terapia-alvo tem mostrado altas taxas de resposta e uma rápida indução de resposta, estando associados ao desenvolvimento de tumores cutâneos como o CEC.
