Resumo: Objective: To analyze the risk factors for death in patients diagnosed with COVID-19 in a triple international border municipality. Methods: Retrospective, analytic-descriptive cohort study conducted with patients confirmed for COVID-19 in Foz do Iguaçu, Paraná, Brazil, in the period from March 2020 to December 2021. The outcome variable of the study consisted of cure vs. death for COVID-19. The exposure variables included demographic data, number and typology of comorbidities. Data were analyzed using absolute and relative frequency distribution. In the analysis of factors associated with death from COVID-19, the Risk Ratio (RR) and its respective confidence intervals were calculated, with statistical significance of 95%. Results: There were 38,814 positive notifications, of which 1,139 (2.9%) died and 37,675 (97.1%) were cured. Death was associated with male gender, age groups over 40 years, race / color brown, and patients with comorbidities, with higher RR for cancer, cardiovascular disease, hypertension, kidney disease, neurological disease, diabetes mellitus, immunodeficiency, obesity, liver disease, lung disease, HIV infection and smoking. Furthermore, the risk of death increased substantially as the multiplicity of comorbidities increased. Conclusions: The profile of mortality by COVID-19 was similar to the state and national literature, in which male patients, with comorbidities and over 40 years of age accounted for the highest number of fatalities.
