INTERRUPÇÃO DA GESTAÇÃO NOS CASOS PREVISTOS EM LEI: CONSTRUÇÃO E VALIDAÇÃO DE UM PROTOCOLO DE SERVIÇO

Resumo: The interruption of pregnancy, in an ethical approach and with reflection on the legal aspects, has as its guiding principles equality, freedom, dignity and humanized attention to women. Abortion is the process of interrupting the gestation of fetuses up to 20 or 22 weeks with a predicted weight of up to 500 grams, which can be spontaneous (natural) or induced (provoked). Worldwide, it is estimated that around 35 abortions occur annually for every 1,000 women between 15 and 44 years age. The objective was to develop and validate a service Protocol for termination of pregnancy for cases provided by the law to be used by a university hospital in the interior of Rio Grande do Sul, which was in the process of receiving authorization to perform the procedures. The methodological study for content validation was used as a method, which is part of the research line of comprehensive care for maternal, neonatal and child health at the Universidade Franciscana (UFN). Initially, there was a literature review and document analysis regarding the termination of pregnancy. Technical visits and interviews were also carried out with professionals working in the obstetric sector, both in the hospital that will use the instrument and in hospitals in Rio Grande do Sul that are already qualified. With that, the elaboration of the Protocol began, which purpose was to standardize and qualify the service and care provided in the reception of women to carry out the termination of pregnancy provided by the law. This product is aimed at medical professionals, nurses, nursing technicians, physiotherapists, psychologists and social workers working in an obstetric center of a medium sized hospital in the interior of Rio Grande do Sul. For the construction of the protocol, the flowchart suggested by the institution in which it will be used was followed. After the formulation, layout and review of the Protocol by the researcher, the second stage of the study was carried out, the validation of this protocol. For this, it was referred to professionals from the matrix support team in sexual violence at the institution where the researcher works and specialists from hospitals qualified to terminate pregnancies in the cases provided by the law, in Rio Grande do Sul. Among the results of the document analysis, it was found that there are several materials that guide the termination of pregnancy in cases provided by the law in Brazil. However, there is a precariousness of updated data on the cases in which the procedure was performed. The integrative literature review showed that several countries have laws that regulate abortion, but most countries have abortion laws linked to the penal code, a fact that brings many stigmas both for the woman who needs the procedure and for the health professionals involved. In addition, it was found that the functioning of pregnancy termination services in different hospitals authorized by the Ministério da Saúde does not have a standard and that each service develops a service flow according to its reality, public served and professionals working. In the validation process with hospital professionals, the content validation indexes regarding objectives, content, language, relevance, functionality and usability ranged from 0.75 to 1, which indicated an overall positive evaluation of the service protocol. Content analysis of these suggestions by professionals regarding the document revealed seven categories: Time, Care, Team, Law/Police, Difficulties, Flows and Others. Such categories were submitted to external judges, whose concordance rate was 80.76%. The final version, after the suggestions by professionals, will be made available on the hospital's internal network and will be available to employees for analysis and suggestions before the final implementation. The product developed in this dissertation is of the Protocol type (T1=100 points by CAPES), it has high relevance and applicability, since it arose from a demand of the service itself. Likewise, as the researcher works at the institution where the Protocol was proposed, she will be able to monitor and evaluate its use in the periodic meetings of the multidisciplinary teams with the management. As final considerations, it should be noted that the study and the product originated and validated from it is a powerful device for changes in the obstetric center of the institution where it will be instituted, benefit for the women assisted, reduction of exposure and risk of revictimization. Thus, the product enabled the inclusion of managers and a multidisciplinary team. This could benefit women, adolescents and children who need this service.
