Resumo: ASSESSMENT OF THE QUALITY OF THE MANAGEMENT OF THE ORAL HEALTH SERVICES IN PRIMARY HEALTH CARE IN MINAS GERAIS: AN EVALUATION OF THE DIMENSIONS INTERSETORIAL ACTION, INFRASTRUCTURE, HUMAN RESOURCES AND POPULAR PARTICIPATION The strengthening of the basic care through programs as Brazil sorridente contributed to improve the population oral health condition. The increasing of 57 percent in oral health teams in the country in the last decade and the financing of almost one billion reais per year paid by the government reinforces the strengthening aims. To evaluate these public and universal services is fundamental to determine a resource equanimity allocation and their qualifications. This study described the care quality in oral health, considering the management of these services under the dimensions of intersectoral action, popular participation, human resources and infrastructure. It checked differences according to the population size; socioeconomic and oral health services indicators of the municipality, the managers profile and characteristics of oral health services management. The study was delineated as ecologic, carried out by 227 municipalities of Minas Gerais. The primary data were collected by electronic questionnaire given by oral health managers of the municipalities, using the Form SUS platform and secondary data of official databases like DATA SUS, IBGE. It was used the evaluation theoretical model proposed by Colussi (2010), qualifying the management according to the mentioned dimensions. Each sub-dimension was evaluated by four indicators (relevance, effectiveness, effectivity and efficiency). Municipalities with indicator lower than 25 percent were considered qualitatively unsatisfactory about the sub-dimension evaluated. Associations were analyzed by Pearson chi-square test or exact of Fisher and by logistic regression model. In general, the results demonstrate that municipalities with population smaller than 5,000 inhabitants present higher averages of the indicators in the four sub-dimensions. Municipalities whose managers were graduated 11 years ago or more, presented higher frequency of unsatisfactory oral health management, in the dimension intersectoral action. Municipalities with greater population size present 7.4 times more chances to present unsatisfactory management in human resource dimension and municipalities over 50,000 inhabitants and Gini over 0.55 have 3.2 times more chances of unsatisfactory quality in infrastructure dimension when compared with smaller municipality. Municipality with greater investment in health primary care present lower frequency of unsatisfactory management in this dimension. The oral health management quality changed according to the municipality population size, having, in general, better results observed in smaller municipalities. Management quality changed also according to the percentage of financial resources invested in HPC, and the graduation time of the oral health manager. The improvement of health service managers with more training time, greater access to fluoridation and the monitoring of their contents in the water supply, greater access by the population to the professional dentist with working hours compatible with the population ascribed as well as the provision of oral health services in the same proportionality of existing health units and reduction of social inequality may influence the improvement of the quality of management of oral health services offered in the municipalities of Minas Gerais.
