Removal of a separated endodontic instrument in the mesiolingual canal of a lower second molar: case report

Summary: | During endodontic treatment, incidents such as instrument fracture inside the root canals can happen. Such complications are caused by torsion or fatigue of the instrument. Therapeutic alternatives include chemical-mechanical preparation up to the fractured fragment; the chemical-mechanical preparation surpassing the fragment; or removal of the instrument via the canal or by apical microsurgery. This paper aims to report the case of a patient referred for endodontic treatment of tooth 47, presenting a fractured endodontic instrument in one of the canals. It was described by the first operator that, during the chemical-mechanical preparation, a reciprocating instrument fractured in the mesiolingual canal. After clinical and radiographic examinations, it was decided to include in the treatment planning the attempt to remove the fragment via the canal, with the aid of operative microscopy and ultrasonic inserts. During the process, the fractured instrument broke again. As its visualization was still maintained, the removal attempt was continued. After the successful procedure, endodontic treatment was performed and completed. After a five-month observation period, the patient remained asymptomatic. Removal of a fractured endodontic instrument via canal is generally the choice option for such complications, as the shaping and disinfection of the canals are more efficient, enabling a high rate of treatment success. However, the professional must pay attention to avoid further accidents or procedural errors. It was concluded that the removal of the endodontic instrument fragment was performed satisfactorily, being a valid attempt for greater predictability of endodontic treatment, when the benefits of the procedure outweigh the risks. |

Tales complicaciones son causadas por torsión o fatiga del instrumento. Las alternativas terapéuticas incluyen preparación químico-mecánica hasta el fragmento fracturado; preparación químico-mecánica superando el fragmento; o extracción del instrumento a través del canal o por microcirugía apical. Este trabajo reporta el caso de paciente remitido para tratamiento endodóntico del diente 47, presentando instrumento endodóntico fracturado en uno de los conductos. Fue descrito por el primer operador que, durante la preparación químico-mecánica, se fracturó un instrumento en el canal mesiolingual. Después de los exámenes clínicos y radiográficos, se decidió intentar retirar el fragmento a través del canal, con ayuda de microscopía quirúrgica e insertos ultrasónicos. Durante el proceso, el instrumento fracturado rompió de nuevo. Como aún se mantenía su visualización, se continuó con el intento de extracción. Después del procedimiento exitoso, se realizó y completó el tratamiento endodóntico. Con un período de observación de cinco meses, el paciente permaneció asintomático. La extracción de un instrumento endodóntico fracturado a través del canal es generalmente la opción de elección para tales complicaciones, ya que la conformación y desinfección de los canales son más eficientes, lo que permite alta tasa de éxito del tratamiento. El profesional debe prestar atención para evitar más errores de procedimiento. Se concluyó que la extracción del fragmento del instrumento endodóntico se realizó satisfactoriamente, siendo un intento válido para mayor previsibilidad del tratamiento endodóntico, cuando los beneficios del procedimiento superan los riesgos.Durante o tratamento endodôntico, incidentes, como fratura de instrumento no interior dos canais radiculares, podem acontecer. Tais complicações são ocasionadas pela torção ou por fadiga do instrumento. As alternativas terapêuticas incluem o preparo químico-mecânico até o fragmento fraturado; o preparo químico-mecânico ultrapassando o fragmento; ou a remoção do instrumento via canal ou por microcirurgia apical. Este trabalho tem por finalidade relatar o caso de paciente encaminhado para tratamento endodôntico do dente 47, apresentando instrumento endodôntico fraturado em um dos canais. Foi descrito pelo primeiro operador que, durante o preparo químico-mecânico, ocorreu a fratura de instrumento reciprocante no canal mésio-lingual. Após exames clínico e radiográfico, optou-se por incluir no planejamento do tratamento a tentativa de remoção do fragmento via canal, com auxílio de microscopia operatória e insertos ultrassônicos. Durante o processo, o instrumento fraturado rompeu-se novamente. Como ainda se matinha sua visualização, continuou-se a tentativa de remoção. Após o procedimento bem-sucedido, o tratamento endodôntico foi executado e finalizado. Com um período de observação de cinco meses, o paciente permanecia assintomático. A remoção de instrumento endodôntico fraturado via canal é, geralmente, a opção de escolha para tais complicações, já que a modelagem e a desinfecção dos canais são mais eficientes, possibilitando uma alta taxa de sucesso do tratamento. No entanto, o profissional deve atentar-se para evitar novos acidentes, ou erros de procedimento.
