MTA in the treatment of furcation perforation in upper molar under operative microscopy: case report

Summary: During endodontic procedures, the occurrence of iatrogenesis can compromise the success of the treatment and knowing how to conduct them is essential for a favorable outcome. Endodontic perforation is a communication between the pulp space and the external surface of the tooth. The most suitable material for sealing perforations is Mineral Trioxide Aggregate (MTA), a bioceramic capable of inducing the formation of mineralized tissue. The use of loupes or operating microscope is an important tool in the management of complex cases, as it allows magnification and illumination of the operating field. The objective of this study is to report a case of a patient submitted to endodontic treatment and sealing of furcation perforation, with the aid of operative microscopy, evidencing the use of MTA. The patient was referred to Faculdade São Leopoldo Mandic for endodontic treatment of tooth 26, which included sealing a perforation and locating the mesiobuccal canal. Chemical-mechanical preparation of root canals was performed with ProDesign Logic instruments, 2% chlorhexidine gel, and distilled water as irrigant. The sealing of the perforation was performed with MTA Angelus. After a period of three months, the patient remained asymptomatic. It was concluded that the treatment was satisfactorily completed, considering the complexity of the case, influenced by patient's age, the difficult access and direct visualization of the tooth and the presence of complications. The use of an operating microscope provided field magnification and illumination, necessary for the detection of the mesiobuccal canal and greater accuracy in the application of the repair material.

La perforación endodóntica es una comunicación entre el espacio pulpar y la superficie externa del diente. El material más adecuado para el sellado de perforaciones es el Agregado de Trióxido Mineral (MTA), una biocerámica capaz de inducir la formación de tejido mineralizado. El uso de lupas o microscopio quirúrgico es una herramienta importante en el manejo de casos complejos. El objetivo de este trabajo es reportar un caso de paciente sometido a tratamiento endodóntico y sellado de perforación en furca, con ayuda de microscopía operatoria, evidenciando el uso de MTA. El paciente fue remitido a la Faculdade São Leopoldo Mandic para tratamiento de endodoncia del diente 26, que incluyó el sellado de perforación y la localización del canal mesiovestibular. La preparación químico-mecánica de los conductos radiculares se realizó con instrumentos ProDesign Logic, gel de clorhexidina 2% y agua destilada como irrigante. El sellado de la perforación se realizó con MTA Angelus. Después de un período de tres meses, el paciente se mantuvo asintomático. Se concluyó que el tratamiento fue completado satisfactoriamente, considerando la complejidad del caso, influenciada por la edad del paciente, el difícil acceso y visualización directa del diente y la presencia de complicaciones. El uso de un microscopio operatorio proporcionó magnificación e iluminación de campo, necesarias para la detección del canal mesiobucal y mayor precisión en la aplicación del material de reparación.Durante procedimentos endodônticos, a ocorrência de iatrogenias pode comprometer o sucesso do tratamento e saber conduzi-las é essencial para um desfecho favorável. O material mais indicado para o selamento de perfurações é o Agregado Trióxido Mineral (MTA), biocerâmico com capacidade de indução da formação de tecido mineralizado. O uso de lupas ou do microscópio operatório é uma importante ferramenta na condução de casos complexos, por possibilitar magnificação e iluminação do campo operatório. O objetivo deste trabalho é relatar um caso de paciente submetido ao tratamento endodôntico e vedamento de perfuração em furca, com o auxílio de microscopia operatória, evidenciando o uso do MTA. O paciente foi encaminhado à Faculdade São Leopoldo Mandic, para tratamento endodôntico do dente 26, que incluía vedamento de perfuração e localização do canal mésio-vestibular. O preparo químico-mecânico dos canais radiculares foi realizado com instrumentos ProDesign Logic, gel de clorexidina 2%, e água destilada como irrigante. O selamento da perfuração foi realizado com MTA Angelus. Após um período de três meses, o paciente encontrava-se assintomático. Concluiu-se que o tratamento foi finalizado satisfatoriamente, tendo-se em vista a complexidade do caso, influenciada pela idade do paciente, pelos difíceis acesso e visualização direta do dente e pela presença de complicações.
