Estudos sugerem que o HEV pode influenciar negativamente o curso da DHC, com maior risco de evolução para cirrose. Dessa forma, o objetivo do estudo foi estimar a prevalência do anticorpo anti-HEV (IgG) em pacientes com DHC e descrever os dados demográficos e fatores de risco para HEV, além de parâmetros clínico-laboratoriais e ultrassonográficos. Para isso, foi desenvolvido um estudo transversal que incluiu 227 pacientes com DHC atendidos no ambulatório de Hepatologia do Hospital das Clínicas da UFPE, de junho de 2022 a março de 2023. Os pacientes realizaram dosagem das enzimas hepáticas, plaquetas, INR e albumina, além do anti-HEV IgG e, nos casos positivos, pesquisa do HEV-RNA. Também foi realizada ultrassonografia do abdome superior. Para a análise da comparação entre as variáveis categóricas foi utilizado o teste Qui-quadrado ou o teste Exato de Fisher. A comparação entre as médias das variáveis com distribuição normal foi realizada através do teste t não pareado, e a comparação entre as medianas das variáveis não paramétricas, através do teste de Mann-Whitney. Para avaliar a associação entre exposição e desfecho foi utilizada a razão de prevalência e o intervalo de confiança de 95%. Foram considerados estatisticamente significantes os resultados com valor de p < 0,05. Foram incluídos 227 pacientes (50 com hepatite B, 49 com doença hepática gordurosa não alcoólica, 33 com hepatite C, 17 com doença hepática relacionada ao álcool, 16 com esquistossomose e 62 com doença mista), 55,5% eram do sexo feminino, com média de idade de 57 ± 13 anos; 37,9% apresentavam cirrose hepática. Sete pacientes (3,08%) apresentaram anti-HEV positivo e em todos o HEV-RNA foi negativo. A frequência do anti-HEV IgG foi maior em pacientes com contato com porcos, com ginecomastia, menor contagem de plaquetas, maiores valores de APRI e FIB-4, e esplenomegalia pelo ultrassom. Embora a prevalência do anti-HEV em pacientes com DHC tenha sido baixa neste estudo, observou-se o anticorpo mais frequentemente nos casos com história de contato com porcos e indícios clínico-laboratoriais ou de imagem de doença hepática mais avançada.The seroprevalence of hepatitis E virus (HEV) in patients with chronic liver disease (CLD) is hardly known in Brazil. Studies suggest that HEV may negatively influence the course of CLD, with a higher risk of progression to cirrhosis. Thus, the objective of the study was to estimate the prevalence of anti-HEV antibody (IgG) in patients with CLD and to describe demographic data and risk factors, as well as clinical-laboratory and ultrasound parameters. For this, a cross-sectional study was developed that included 227 patients with CLD followed at the Hepatology outpatient clinic of the Hospital das Clínicas da UFPE, from June 2022 to March 2023. The patients were investigated for the activity of liver enzymes, platelets, INR and albumin, in addition to anti-HEV IgG and, in positive cases, for HEV-RNA. Ultrasonography of the upper abdomen was also performed. For the analysis of the comparison between the categorical variables, the chi-square test or Fisher's exact test were used. The comparison of the means of the variables with normal distribution was performed using the unpaired t test, and the comparison between the medians of the nonparametric variables using the Mann-Whitney test. To assess the association between exposure and outcome, the prevalence ratio and 95% confidence interval were used. Results with a p value < 0.05 were considered statistically significant. A total of 227 patients were included (50 with hepatitis B, 49 with non-alcoholic fatty liver disease, 33 with hepatitis C, 17 with alcohol-related liver disease, 16 with schistosomiasis and 62 with mixed disease), 55.5% were female, with a mean age of 57 ± 13 years; 37.9% had liver cirrhosis. Seven patients (3.08%) were anti-HEV positive and the HEV-RNA was negative in all of them. The frequency of anti-HEV IgG was higher in patients with contact with pigs, with gynecomastia, lower platelet counts, higher APRI and FIB-4 values, and splenomegaly on ultrasound.
