Comprometimento motor e funcionalidade de crianças e adolescentes com paralisia cerebral em atendimento fisioterapêutico: um estudo transversal

Summary: Introduction: Cerebral palsy (CP) is a disease characterized as a non-progressive neurological condition deriving from injury to the immature brain, which can generate motor, cognitive, sensory, perceptual, on communication and behavioral dysfunctions. In this way, it affects the motility and functionality of the affected individuals. The child with CP goes through stages of physiotherapeutic treatment and usually remains a good part of childhood and adolescence in Specialized Rehabilitation Services. Objective: To identify the motor impairment and functionality of users with cerebral palsy treated at the Child Physiotherapy Service (SFI) of the Lauro Wanderley University Hospital (HULW). Methodology: The present study is descriptive and cross-sectional, with a quantitative approach, consisting of 14 patients diagnosed with CP, selected in a non-probabilistic manner, attended at the HULW Child Physiotherapy Service. In addition, the Gross Motor Function Classification System (GMFCS) was applied to obtain the level of motor impairment (I to V) and the Pediatric Evaluation of Disability Inventory (PEDI), functional skills and caregiver assistance. Descriptive statistics were performed through frequency (mean and standard deviation), scores and scores of GMFCS and PEDI. The data collected were organized and tabulated using the Microsoft Office Excel 2010 program. Results: Sample participants were 64% male and 36% female. They presented age ranging from 2 to 15 years, 50% being up to 4 years of age, followed by 29% between 9 and 12 years. Regarding the type of CP, the majority (36%) had spastic diplegia. Concerning GMFCS, 9 participants (65%) reached between levels IV and V, which revealed an important motor impairment. It was observed that individuals whose ages were highest had the highest level in GMFCS, and an inversely proportional relationship between GMFCS level and continuous mobility assessment scores. Conclusion: The present study indicates levels of motor impairment and functionality in the CP, however, the most important aspect to highlight is that it can visualize through the scales used the skills and potentialities of the participants, which can further collaborate in the process of rehabilitation they are in.

Desta forma, afeta a motricidade e funcionalidade dos indivíduos acometidos. A criança com PC passa por etapas de tratamento fisioterapêutico e comumente permanece boa parte da infância e adolescência em Serviços Especializados de Reabilitação. Objetivo: Identificar o comprometimento motor e a funcionalidade dos usuários com paralisia cerebral atendidos no Serviço de Fisioterapia Infantil (SFI) do Hospital Universitário Lauro Wanderley (HULW). Metodologia: O presente estudo é de caráter descritivo e transversal, com abordagem quantitativa, sendo composto por 14 pacientes diagnosticados com PC, selecionados de forma não probabilística, atendidos no Serviço de Fisioterapia Infantil do HULW. Foram coletadas informações de identificação e referente ao quadro clínico, além da aplicação do Gross Motor Function Classification System- GMFCS para obter o nível de comprometimento motor (I à V) e o Pediatric Evaluation of Disability Inventory- PEDI para obter os escores contínuos referentes às habilidades funcionais e assistência do cuidador. A estatística descritiva foi realizada por meio da verificação das frequências (médias e desvio padrão), pontuações e escores da GMFCS e PEDI. Os dados coletados foram organizados e tabulados utilizando o programa Microsoft Office Excel 2010. Resultados: Os participantes da amostra eram 64% do sexo masculino e 36 % sexo feminino. Apresentaram idade variando de 2 à 15 anos, sendo a maioria (50%) com até 4 anos de idade, seguida de 29% entre 9 e 12 anos. Quanto ao tipo de PC, a maioria (36%) apresentava diplegia espástica, no que se refere ao GMFCS 9 participantes (65%) alcançaram entre os níveis IV e V o que revelou importante comprometimento motor. Observou-se que os indivíduos cujas idades eram mais elevadas possuíam o nível mais elevado no GMFCS, e uma relação inversamente proporcional entre o nível da GMFCS e os escores contínuos avaliação da mobilidade.
