spelling PAPEL DO MITOTANO COMO TERAPÊUTICA ADJUVANTE NO TRATAMENTO DO CARCINOMA ADRENOCORTICALRole of mitotane as adjuvant therapy in adrenocortical carcinoma treatmentCarcinoma adrenocorticalMitotanoQuimioterapiaGlândula suprarrenalAdrenal GlandAdrenocortical CarcinomaMitotaneChemotherapyTrabalho Final do Mestrado Integrado em Medicina apresentado à Faculdade de MedicinaAdrenocortical Carcinoma is a rare malignant endocrine tumor (reported incidence of 2 cases per million per year), but with high mortality rate (overall 5-yearl survival rate between 16-47%).Its treatment requires a multidisciplinary approach involving surgical resection, medical adjuvant therapy, chemotherapy, and radiotherapy.Mitotane is a derivative of dichlorodiphenyltrichloroethane used in complex Cushing Syndrome patients; due to its steroidogenesis inhibitor and adrenolytic effect, it’s also approved in advanced adrenocortical carcinoma treatment or with high recurrence risk: ENSAT III or higher, Ki67>10%, R1 or Rx resection.When feasible, surgery is the best treatment, but even in these cases there’s a high recurrence rate; being so, adjuvant therapy with Mitotane might be associated. Mitotane is also approved in metastatic disease, controlling tumor progression and, if clinically relevant, the hormonal secretion.However, Mitotane treatment still raises great controversy dependent of its toxicity; it causes adrenocortical insufficiency (affects predominantly zona fasciculata and zona reticularis) which requires replacement corticotherapy in higher doses than other adrenocortical insufficiencies; thyroid disfunction that mimics a hypothyroidism; in men it inhibits testosterone secretion; in high doses it can affect the central nervous system leading to neuromuscular symptoms; hematological changes such as anemia or thrombocytopenia; hepatotoxicity, and others.Current studies aren’t consistent about Mitotane’s impact in disease-free survival, but it is usually limited, requiring association of other therapeutic options.Currently there isn’t an alternative adjuvant therapy for Mitotane in adrenocortical carcinoma treatment, being mandatory to optimize the knowledge about the tumor, the drug, and its effects.O carcinoma adrenocortical é uma neoplasia endócrina maligna rara (incidência estimada de 2 casos por milhão por ano), mas com elevada taxa de mortalidade (sobrevida média aos 5 anos entre 16 e 47%).O seu tratamento requer uma abordagem multidisciplinar, envolvendo ressecção cirúrgica, terapêutica médica adjuvante, quimioterapia e radioterapia. O Mitotano é um derivado do dicloro-difenil-tricloroetano usado no tratamento de casos particularmente complexos de síndrome de Cushing; devido ao seu efeito inibidor da esteroidogénese e adrenolítico também está aprovado no tratamento do carcinoma adrenocortical avançado ou com elevado risco de recorrência: estádio ENSAT III ou superior, Ki67>10%, ressecção R1 ou Rx.Quando exequível, a cirurgia é o tratamento de eleição, mas mesmo nestes casos a taxa de recorrência é elevada, sendo que terapia adjuvante com Mitotano pode ser associada.
