A gastrectomia associada a adequada linfadenectomia continua a ser o único tratamento potencialmente curativo. Este estudo tem como objetivo avaliar e comparar resultados entre as abordagens laparoscópica e laparotómica, no sentido de aferir se é possível obter resultados oncológicos semelhantes com um acesso menos invasivo.Materiais e métodos: Selecionaram-se 167 doentes (com idade média de 73,5 anos e predomínio do sexo masculino) submetidos a intervenção cirúrgica curativa por carcinoma gástrico no serviço de Cirurgia Geral do Centro Hospitalar e Universitário de Coimbra, no período compreendido entre janeiro de 2017 e abril de 2022. Destes doentes, 49,7% foram submetidos a laparotomia e 50,3% a laparoscopia. Em 85% dos casos foi realizada gastrectomia subtotal, em 10,2% gastrectomia polar superior e em 4,8% gastrectomia total. Com recurso a análise estatística descritiva e inferencial analisaram-se dados pré, intra e pós-operatórios, recolhidos dos processos clínicos dos doentes.Resultados: A técnica laparoscópica apresentou menor duração de internamento (16,27 vs 17,48 dias), um maior número de gânglios excisados (20,99 vs 18,47), menos casos de recidiva global (8 vs 18) mas maior tempo operatório (226,94 vs 203,61 minutos). Em ambas as técnicas cirúrgicas ocorreram complicações, sendo a mais frequente a fístula da anastomose. Em termos de tipo de linfadenectomia e ocorrência de mortalidade pós-operatória, os resultados de ambas as técnicas foram comparáveis. Discussão: As características da nossa população eram similares à restante população ocidental. Neste estudo, a laparoscopia tornou-se a abordagem predominante no tratamento do carcinoma gástrico ressecável, à semelhança da grande parte do mundo. Esta via de abordagem apresentou-se comparável e em certas variáveis superior à laparotomia, nomeadamente na duração de internamento, número de gânglios excisados e ocorrência de recidiva em doentes com estadio III. Conclusão: Neste estudo, a gastrectomia laparoscópica mostrou-se uma técnica segura e mais eficaz em vários aspetos abordados, em alternativa à laparotómica.Introduction: Gastric cancer represents the fifth most common cancer and the third leading cause of cancer death worldwide. Gastrectomy associated with adequate lymphadenectomy still is the only potentially curative treatment. This study aims to evaluate and compare results between laparoscopy and laparotomy, to assess whether similar oncological results can be obtained with a less invasive access.Methods: 167 patients with mean age 73.5 years and male predominance, submitted to curative surgery for gastric cancer at the General Surgery department of the Centro Hospitalar e Universitário de Coimbra between January 2017 and April 2022, were selected. 49.7% underwent laparotomy and 50.3% laparoscopy. Subtotal gastrectomy was performed in 85% of the cases, superior polar gastrectomy in 10,2% and total gastrectomy in 4,8%. With descriptive and inferential statistical analysis, pre, intra and postoperative data were analysed, obtained from the patient’s clinical files. Results: The laparoscopic technique had a shorter hospital stay (16.27 vs 17.48 days), a greater number of excised lymph nodes (20.99 vs 18.47), fewer cases of global recurrence (8 vs 18) but longer operative time (226 .94 vs 203.61 minutes). Complications occurred in both surgical techniques, the most frequent being anastomotic fistula. In terms of the type of lymphadenectomy and occurrence of post-operative mortality, results of both techniques were comparable.Discussion: The characteristics of our population were similar to the rest of the western population. In this study, laparoscopy became the predominant approach in the treatment of resectable gastric carcinoma, as in most of the world. This approach was comparable and in certain variables superior to laparotomy, namely in the length of stay, number of excised lymph nodes and evidence of recurrence in patients with stage III.
