O seu diagnóstico imagiológico requer habitualmente a realização de tomografia computorizada (TC), um método dispendioso e que obriga à exposição do doente a radiação. Assim, torna-se relevante otimizar o uso desta técnica, selecionando os doentes que dela possam beneficiar, nomeadamente identificando corretamente os casos com necessidade de terapêutica invasiva por contraponto aos casos que apenas necessitam de tratamento conservador.Objetivo: Pesquisar a correlação entre marcadores analíticos frequentemente requisitados em contexto de urgência (proteína C reativa, contagem de leucócitos e índice neutrófilo-linfócito) e os achados de TC que indicam a necessidade de tratamento invasivo segundo as últimas guidelines da World Society of Emergency Surgery. Determinar um cut-off desses marcadores que se correlaciona com uma maior probabilidade de complicações em TC.Métodos: Estudo retrospetivo descritivo, utilizando uma amostra de pacientes diagnosticados com diverticulite aguda do cólon sigmóide e estudados por tomografia computorizada em contexto de urgência no Centro Hospitalar Universitário de Coimbra, entre 21 de março de 2022 e 21 de março de 2023. Análise estatística dos dados com recurso ao teste U de Mann-Whitney e a curvas Receiver operating characteristic.Resultados: Amostra de 93 indivíduos com uma idade média de 61,77 anos, 53,8% do sexo masculino. Foram encontradas diferenças significativas entre os grupos de pacientes com diverticulite clinicamente leve e com diverticulite clinicamente grave nos valores de proteína C reativa e no índice neutrófilo-linfócito. A curva Receiver Operating Characteristics para o primeiro marcador teve uma área debaixo da curva de 88,4%, com um valor de 16,00mg/dL a demonstrar uma sensibilidade de 77,78% para a deteção de diverticulites clinicamente graves com necessidade de terapêutica invasiva. Para o índice neutrófilo-linfócito o valor da área debaixo da curva foi de apenas 66,1%, sendo então este um marcador não satisfatório para discernimento dos dois grupos pretendidos.Discussão e Conclusão: Foi possível encontrar uma correlação significativa entre o valor de proteína C reativa e a presença de diverticulite com critérios para terapêutica invasiva por TC. Contudo, o seu valor não possui uma sensibilidade suficientemente elevada para fundamentar a dispensa de TC em casos de suspeita desta patologia,mantendo-se a avaliação imagiológica com este instrumento uma parte fundamental do diagnóstico.Introduction: Acute diverticulitis is an acute abdomen condition with growing incidence in the world. Its imagiological diagnostic usually requires the use of computed tomography (CT), an expensive imaging method that requires exposure of the patient to radiation. Therefore, it becomes relevant to optimize the use of this technique, selecting the patients that might benefit from it, namely by correctly identifying the cases with need for invasive therapy, as opposed to the ones that only require conservative treatment.Objectives: To search for a correlation between analytical biomarkers frequently requested in an emergency context (such as C reactive protein, leukocyte count and neutrophil-lymphocyte ratio) and the CT findings that indicate a need for invasive treatment according to the latest guidelines by the World Society of Emergency Surgery. To determine a cut-off value of these markers that correlates with a higher likelihood of complications found in CT.Methods: Retrospective chart review using a sample of patients diagnosed with acute sigmoid diverticulitis in an emergency setting in the Centro Hospitalar Universitário de Coimbra between the 21st of march of 2022 and the 21st of march of 2023. Statistical analysis of the data made using the Mann-Whitney U test and Receiver operating characteristic curves.Results: A sample of 93 individuals was obtained, with a medium age of 61,77 years, 53,8% of whom were male. Statistical differences between the groups with clinically light diverticulitis and clinically severe pathology were identified for C reactive protein values and neutrophile-lymphocyte ratio. The Receiver Operating Characteristic curve for the first marker had an area under the curve of 88,4%, proving to be a good predictor of severe diverticulitis. A C reactive protein value of 16,00mg/dL was shown to have a sensitivity of 77,78% in detecting clinically severe cases with need for invasive treatment. For the neutrophile-lymphocyte ratio, the area under the curve was only 66,1%, with this being a non-satisfactory biomarker for distinguishing both groups.Discussion/Conclusion: It was possible to find a significative correlation between the C reactive protein value and the presence of acute diverticulitis fulfilling the criteria for invasive therapy.
