PATERNALISM OF THE CAREGIVER AND AUTONOMY OF THE ELDERLY IN THE HOME CARE SERVICE

Summary: | Due to the drop in birth and death rates, global population aging has been increasing on a large scale. In Brazil, it is estimated that, in 2050, the elderly population will total 21.87%. Aging changes epidemiological characteristics and intensifies health services, making it necessary to build care alternatives in the face of these challenges. The care provided to the elderly depends on their physical and psychological conditions and the caregiver's view of aging and their care skills. From this view, paternalistic or elderly-centered care practices may emerge. This study investigated how caregiver paternalism occurs and its repercussions on the autonomy of elderly people monitored by the Home Care Service (SAD) in Fortaleza-CE. In field research with a qualitative and exploratory approach, semi-structured interviews were carried out with 6 elderly people and their caregivers. Data were collected at the patients' homes, between November and December 2023. According to the content analysis coding process by Bardin (2016), 03 thematic categories were obtained: 1) Caregiver's perception of the care provided to the elderly; 2) Paternalistic practices of the caregiver in caring for the elderly; 3) Repercussions of the caregiver's paternalism on the elderly's autonomy. The results confirm a paternalistic care practice characterized by overprotective attitudes, compromising the autonomy of the elderly. This makes us reflect on the importance of care that stimulates the autonomy of the elderly, understanding their subjectivity, preserving their dignity and guaranteeing their quality of life. |

En Brasil, se estima que, en 2050, la población anciana alcanzará el 21,87%. El envejecimiento cambia las características epidemiológicas e intensifica los servicios de salud, haciendo necesario construir alternativas de atención frente a estos desafíos. El cuidado brindado a las personas mayores depende de sus condiciones físicas y psicológicas y de la visión que tiene el cuidador sobre el envejecimiento y sus habilidades de cuidado. Desde esta visión, pueden surgir prácticas de cuidado paternalistas o centradas en las personas mayores. Este estudio investigó cómo ocurre el paternalismo del cuidador y sus repercusiones en la autonomía de las personas mayores atendidas por el Servicio de Atención Domiciliaria (SAD) de Fortaleza-CE. En una investigación de campo con enfoque cualitativo y exploratorio, se realizaron entrevistas semiestructuradas a 6 personas mayores y sus cuidadores. Los datos fueron recolectados en el domicilio de los pacientes, entre noviembre y diciembre de 2023. Según el proceso de codificación del análisis de contenido realizado por Bardin (2016), se obtuvieron 03 categorías temáticas: 1) Percepción del cuidador sobre el cuidado brindado al anciano; 2) Prácticas paternalistas del cuidador en el cuidado del anciano; 3) Repercusiones del paternalismo del cuidador en la autonomía del anciano. Los resultados confirman una práctica de cuidado paternalista caracterizada por actitudes sobreprotectoras, que comprometen la autonomía de las personas mayores. Esto nos hace reflexionar sobre la importancia de un cuidado que estimule la autonomía de las personas mayores, comprendiendo su subjetividad, preservando su dignidad y garantizando su calidad de vida.Devido à queda das taxas de natalidade e mortalidade, o envelhecimento populacional mundial vem aumentando em grande escala. O envelhecimento altera as características epidemiológicas e intensifica os serviços de saúde, sendo necessário construir alternativas de cuidados diante destes desafios. Os cuidados prestados ao idoso dependem das condições físicas e psicológicas deles e a visão do cuidador tem sobre envelhecimento e suas habilidades de cuidado. A partir dessa visão, podem surgir práticas de cuidado paternalistas ou centradas no idoso. Este estudo investigou como ocorre o paternalismo do cuidador e suas repercussões na autonomia dos idosos acompanhados pelo Serviço de Atenção Domiciliar (SAD) de Fortaleza-CE. Na pesquisa de campo de abordagem qualitativa e exploratória, foram aplicadas entrevistas semiestruturadas em 06 idosos e seus cuidadores. Os dados foram coletados na residência dos pacientes, entre novembro e dezembro de 2023. Conforme o processo de codificação da análise de conteúdo de Bardin (2016), foram obtidas 03 categorias temáticas:1) Percepção do cuidador sobre o cuidado prestado ao idoso; 2) Práticas paternalistas do cuidador no cuidado ao idoso; 3) Repercussões do paternalismo do cuidador na autonomia do idoso. Os resultados constatam uma prática de cuidado paternalista caracterizada por atitudes de superproteção, comprometendo a autonomia do idoso.
