Resumo: | Population aging has brought complex challenges for the public health service of the country, among them, increased spending for care for prevalent diseases in old age. Because of the multiplicity and chronicity of comorbidities that affect them, elderly people consume a large number of drugs which, along with the physiological changes of aging, increase the risk of inappropriate use of medicines, and consequently, the occurrence of adverse reactions. Among the indicators of quality of care related to drug therapy the polypharmacy, the use of inappropriate drugs for the age and the occurrence of adverse drug reactions stands out. The objective of this study was to determine the profile of pharmacotherapy prescribed to a population of elderly residents in Long-Stay Institutions for the Elderly (LSIE) through the analysis of these indicators. We conducted a literature review in which explained the major physiological changes that occur with aging and its effects on pharmacokinetic and pharmacodynamic mechanisms, and describes the main implications of the use of drugs for seniors - polypharmacy and the use of potentially inappropriate medications, their definitions, evaluation criteria and prevalence. To verify the use of inappropriate medications and adverse reactions to drugs, we performed a cross-sectional study, with data collected through analysis of medical records of elderly residents in six LSIE of four cities of the state of São Paulo in Brazil, supplemented with interviews of nurses responsible for these patients. All pharmaceuticals prescribed were classified into therapeutic categories, according to its active principle, referencing to the Anatomical-Therapeutic-Chemical. Inappropriate medications were identified and classified using the Beers criteria recently updated by the American Geriatrics Society. For the detection of adverse... |
