Resumo: Reminder therapy has stood out as an innovative approach to promoting health and adherence to healthcare treatments. This therapy consists of using frequent reminders, usually through electronic devices, to reinforce desirable health behaviors, especially in young people who use fixed orthodontic appliances and face difficulties in oral hygiene. Thus, the objective of this systematic review was to evaluate the scientific evidence related to the influence of reminder therapy on oral health and adherence to treatment in patients undergoing orthodontic treatment with fixed appliances. Randomized clinical trials that evaluated reminder therapy associated with the outcomes of gingival index, bacterial plaque index, white spot and treatment adherence with patients undergoing orthodontic treatment with fixed appliances were included. No restrictions were imposed regarding language and year of publication. A broad search was conducted in seven main databases: EMBASE, LILACS, BBO, CUMED, MEDLINE, SciELO, Cochrane Library in addition to the Scopus and Web of Science citation bases. Part of the gray literature was captured in the ProQuest and Opengrey databases. The searches were carried out in February 2024. Two reviewers, previously trained, carried out all steps independently. Risk of bias was assessed using the RoB2 tool. A bibliographic analysis was carried out on the density of keywords, journals with the highest number of articles published, their impact factor and the geographic prevalence of eligible studies. Twenty-nine studies carried out between 2004 and 2023 were included in the qualitative synthesis. The total sample included 2,111 individuals. Reminder therapy was effective in reducing plaque index, gingival index, bleeding index, white spot and improving treatment adherence in orthodontic patients with fixed appliances. The studies had a greater concentration on the Asian continent and a peak of publications between 2015 and 2021. The integration of these technologies is effective in improving clinical results during orthodontic treatment. A protocol was carried out and submitted to PROSPERO (CRD42024565710).
