Resumo: Introduction: Onychomycosis affects about 10% of the global population, and its management is challenging, since the treatment is prolonged, has a high cost and presents a risk of systemic toxicity. The high rates of therapeutic failure, recurrence and reinfection are factors directly linked to low penetration and the difficulty of maintaining adequate levels of the drug at the site of infection. Therefore, new techniques, instruments or technologies, such as lasers, that help improve treatment are desirable. Objective: To compare the efficacy of Er: YAG and Nd- YAG lasers alone and/or in association with topical antifungals in relation to monotherapy with topical antifungals in the treatment of onychomycosis. Material and methods: Systematic review of randomized clinical trials conducted in the last 10 years without meta-analysis. Relevant studies were identified through research in databases such as PubMed, Embase and Lilacs, with the keywords "onychomycosis", "drug therapy", "laser". Results: Twenty-one articles were found, of which 6 met inclusion criteria, being analyzed and discussed in full. Conclusion: Combined treatment of LASERS (Er YAG or Nd: YAG) and topical antifungals has shown superior efficacy to topical antifungal monotherapy in the treatment of onychomycosis. However, when we compared laser monotherapy and topical antifungal monotherapy, the results are divergent and inconclusive.
