Prevalência de neuropatia autonômica cardiovascular em pacientes com disglicemia submetidos ao cateterismo coronariano

Resumo: | Introduction: Cardiovascular autonomic neuropathy (CAN) is a clinically significant complication of diabetes mellitus (DM) with association to cardiovascular disease (CVD). This complication might already be present early in the natural history of DM and even in prediabetes (pre-DM). The objective of this study is to estimate the prevalence of CAN in patients with dysglycemia submitted to coronary angiography. Methods: The total of 84 patients with dysglycemia, both type 2 diabetes mellitus (T2DM) or pre-DMsubmitted to coronary angiography were studied by the deep breathing test (E: I), Valsalva maneuver, lying to standing test (30:15 ratio) and orthostatic hypotension test in order to identified CAN. Results: The study group was 63% male,with age 60.5 ± 10.7 yrs, body mass index (BMI) of 27.9± 4.8kg/m2 . The prevalence of coronary artery disease (CAD) was 68% and CAN was 7.7% in pre-DM and 22.2% in subjects with T2DM (p=0.066). The diabetes status, its duration and dyslipidemia showed a trend to be associated with CAN. Subjects with CAN had a similar prevalence of CAD of those without CAN (85% vs. 65%; P=0.159). Those with severe CAN were more frequently T2DM patients with longer disease duration and with higher prevalence of dyslipidemia. Conclusion: The prevalence of CAN in patients with dysglycemia submitted to a coronary angiography was 15%,with the concomitant prevalence of CAD in 85% with no statistical significance between those with pre-DM or T2DM. We found a trend of association between CAN, dyslipidemia and diabetes duration. |
