It is the translation and cultural adaptation of the Functional Outcomes of Sleep Questionnaire (FOSQ10), to the Brazilian Portuguese. The method of translation and adaptation follows those suggested in the literature: two independent translators, first version synthesis committee, back-translation, equivalence evaluation, and pre-test in target population. The phases of the cross-cultural adaptation process were followed and the structures evaluated, obtaining a satisfactory percentage of agreement (≥ 80%). The suggested adjustments were performed in the pre-test version, being considered appropriate in its application, without the need for further modifications. Named as "Questionário sobre os Resultados Funcionais do Sono (FOSQ10)”, as the official Portuguese Version of Brazil. Following the suggested phases for cultural adaptation, FOSQ10 is available in Portuguese from Brazil to test its psychometric qualities.
