Resumo: The epidemiological and demographic transition reflects the worldwide concern with Chronic Noncommunicable Diseases, causing various health complications, including vascular alterations. This work aims to know the perception of users with vascular alterations of a University Hospital in the interior of the state of RS, about their therapeutic itinerary, with a view to contributing to the strengthening of the Health Network of the 4th CRS, RS. This is an exploratory-descriptive study, of qualitative approach with an intentional sample of users with vascular alteration. Data collection occurred between May and October 2018 and the narrative interview was used as a data collection technique. The results obtained through thematic content analysis were organized into two categories: "Link with primary health care" and "Naturalization of payment: time is public, time is private". In conclusion, it was evidenced that it is necessary to reorganize the structure of health services, to provide articulations between specialized services for care for people with vascular alteration, and to problematize the entry of the private sector into the itineraries, in order to contribute to the care of health based on the promotion and direction of comprehensive care.
