Comunicação entre as equipes de atenção básica a partir da referência e contrarreferência de criança/adolescente com doença crônica

Resumo: Introduction: Communication between teams working in primary health care is a fundamental tool for the implementation of the referral and counter-referral system in the care of children/adolescents with chronic disease. Communication contributes to the integrality of the care network, enhancing health actions that will provide continuity of care for this population. Objective: To understand the communication between the multidisciplinary teams of primary care and Family Health, based on the referral and counter-referral system of children/adolescents with chronic disease. Method: Exploratory-descriptive research with a qualitative approach, carried out in Family Health Units in a municipality in Paraíba. Thirteen professionals participated, including a doctor, a psychologist, a nutritionist, two speech therapists, two physiotherapists and six nurses (NASF-AB and ESF). The in-depth interviews took place remotely between September and December 2021 and the data were interpreted by Inductive Thematic Analysis. The project was approved by the Research Ethics Committee, CAAE: 47656821.5.0000.5188 and Protocol nº 4,866,676. Results: Health professionals showed that the communication between the multidisciplinary teams of primary care and family health does not occur in a adequate due to the non-effectiveness of the counter-referral system. The non-completion of the formal counter-referral instrument, added to the accumulation of functions of the professionals of the multiprofessional team, were identified as obstacles that prevent them from providing the necessary support in the family health unit and impair communication between them. Final Considerations: It was identified the need to encourage managers to strengthen communication between teams. As for the members of primary care teams who integrate communication through formal instruments into their work processes, in addition to other mechanisms that increase the bond between them, such as holding team meetings and drawing up a care plan together.
