Resumo: Introduction: The Directly Observed Treatment (DOT) for Tuberculosis (TB) is a strategy carried out in Primary Health Care that contributes to the improvement of the epidemiological indicators of this disease. Policy transfer occurs when a given strategy shifts from one time or space to another in order to achieve better results, taking into account the commitment of all involved. Objective: to analyze the effects of senses produced in managers' discourses on the policy transfer from DOT to TB in counties of the great region of João Pessoa-PB. Materials and Methods: A descriptive, qualitative and discursive approach was carried out between 2017 August and November, through an interview technique, in a semi-directed script for managers of three counties of Greater João Pessoa: Bayeux, Cabedelo and Santa Rita, being three Health Secretaries and three managers responsible for the Tuberculosis Control Program in the respective counties. The empirical material was analyzed through the theoreticalmethodological device of Discourse Analysis (DA) of French pecheutian affiliation. The study was approved by the Research Ethics Committee of the Health Sciences Center of the Federal University of Paraíba (protocol nº 0193/2017 / CEP / CCS / UFPB). Results: Most of the managers' speeches announced beginner proposals for TB control actions, leaving to emphasize the DOT in a second moment, considering that two counties underwent management transition. Some managers outlined the resources and partnerships that exist as facilitators of the process to implement TB actions. As obstacles, the patients' resistance to leaving the state referral unit to be monitored in their county was highlighted, as well as highlighting the professionals' prejudice, stigma and turnover. Through DA, some contradictory meanings were identified, the unspoken and other elements, such as repetitions, paraphrases and metaphors. Silences were also observed, especially regarding DOT. Some managers highlighted the importance of commitment and awareness among all those involved in TB control. Conclusion: It is suggested that the study be continued at a later date in order to better offer subsidies in the evaluation regarding the transfer of DOT policy, especially in the counties that have undergone a transition in management. In addition to highlighting some proposals for strengthening TB control actions, based on the Network for Health Care and Permanent Education, the study invites the reader to reflect on "being subject" and looking at the "Other", based on Levinasian thought.
