Proposta de subconjunto terminológico da Classificação Internacional para a Prática de Enfermagem - CIPE® para hipertensos na atenção básica.

Resumo: | INTRODUCTION: Arterial Hypertension is a multifactorial clinical condition that manifests as a syndrome characterized by the presence of elevated and sustained blood pressure levels, being considered a serious public health problem in Brazil and worldwide. Therefore, hypertensive patients need quality assistance, especially from the nurse whose care must follow every step of the nursing consultation in primary care, having as theoretical framework the Chronic Disease Care Model applied in Nursing associated with the Horta's Basic Human Needs conceptual model. OBJECTIVE: Design an ICNP® Terminological Subset for hypertensive patients monitored in primary health care. METHOD: This is an exploratory descriptive survey conducted in four stages: 1) Collection of relevant terms and concepts regarding the nursing practice related to the client with Arterial Hypertension based on document analysis about Systemic Arterial Hypertension of the Ministry of Health; 2) Formulation of statements the nursing diagnoses/results and interventions as recommended by the International Council of Nurses, based on the reference model for nursing diagnosis and nursing action of the ISO 18.104 and the bank of terms; 3) Validation of statements the nursing diagnoses/outcomes and interventions formulated by a discussion group, with the participation of representative nurses from the Sanitary Districts of João Pessoa s Health Department; and 4) Organization of a ICNP® Terminological Subset for hypertensive patients in primary care including its significance to the nursing practice, of the chosen theoretical model, and of the relationship between the statements of nursing diagnoses/results and interventions. RESULTS: There has been identified 565 relevant terms to the nursing practice in the field of arterial hypertension, which were subjected to cross-mapping with the terms from the 2011 ICNP® 7-Axis Model, that resulted on the Nursing Language Term Bank, with relevant terms to the arterial hypertension field, containing 178 included and 387 terms not included on this classification. The formulation the statements of nursing diagnoses/results and interventions was guided by the recommendations of the International Council of Nurses, finishing with 60 statements of nursing diagnoses/results and 351 statements of nursing intervention. These statements were distributed in accordance with the Chronic Disease Care Model and the Horta s Basic Human Needs Theory. The proposed structure for the ICNP® Terminological Subset for Hypertensive Patients in Primary Care is an instrument to aid nurses in the working process organization concerning hypertensive patients nursing care, as well as a standardization on nursing records produced in nursing consultation. CONCLUSIONS: It is expected that the proposed terminological subset of the International Classification for Nursing Practice - ICNP® for hypertensive patients in primary care turns out to be effective and qualified, and that it may provide an improvement on the assistance records, because it is believed that the results of this research behold a necessity in the nursing practice constructing a terminology that enables an Information Systems for Primary Health Care evolution, as well as an unified nursing language utilization. |
