Subconjunto terminológico da Cipe®, estruturado em ontologia, para o autocuidado da pessoa com estomia de eliminação intestinal

Resumo: | Introduction: Care with people with intestinal elimination stomas aims the rehabilitation by means of self-care. However, there is a lack of knowledge regarding instruments that anchor the systematized care to the specific group of clients, based on a nursing terminology and on a theory that supports self-care, and represented in order to be inserted into health information systems. Aim: To develop a terminological subset of ICNP® for self-care of the person with intestinal elimination stoma, based on the Self-Care Theory and structured in ontology. Methods: Methodological research, developed in four steps based on the Brazilian method for the development of terminological subsets of ICNP®. The first step consisted of identifying terms for the care of the person with the intestinal elimination stoma, from 49 articles on the subject and from nursing records of 39 patients from the care service to people with ostomies of the University Hospital Lauro Wanderley from the Federal University of Paraíba (HULW-UFPB); The second stage was accomplished by crossing the terms identified with the terms of ICNP® Version 2015; The third step involved the construction, from the terms identified and mapped, of statements of nursing diagnosis/outcomes and interventions for the self-care of selected clientele and validation of content of statements by specialists, considering valid those with a degree of agreement greater than or equal to 80%; And a fourth step consisted in structuring the subset, according to the Brazilian method for the development of subsets, based on Orem's theory, and in ontology. The research was approved by the Research Ethics Committee of HULW-UFPB. Results: There were identified 457 relevant terms in the literature and in the clinical records, of which 173 are included in ICNP® Version 2015. There were constructed 110 diagnoses/outcomes, of which 78 (70.9%) were validated by specialists and included in the subset, and 119 interventions, of which 103 (86.5%) were validated and included in the subset. In the subset structured according to the Brazilian method for the development of subsets, 17 nursing diagnoses/outcomes were placed as universal self-care requisites, 40 as self-care requisites related to development, and 13 as self-care requisites related to health alterations of Orem’s theory; And of the nursing interventions, two correspond to the totally compensatory system, four to the partially compensatory system and 97 to the support-education system of Orem's theory. In the subset structured in ontology, the statements were arranged hierarchically in the Brazilian ontology of ICNP®. Conclusions: It is believed that statements validated and based on Orem's theory represent the phenomena and self-care needs of the person with intestinal elimination stoma, and as specific actions to promote self-care of this clientele. It is considered that the developed subset contributes to the systematic collection and documentation of the elements of the nursing practice directed to the person with intestinal elimination stoma. It is suggested the inclusion of the ontology in health information systems, in order to allow its use by nursing professionals. |
