Resumo: Introduction: Research on drug use by elderly people is scarce, so the present study is important in investigating factors that may be linked to drug use by this population. Objective: to investigate the repercussions of drug use on the cognition, mood and family support of elderly people. Methodology: The study was developed in two stages. The first consisted of preparing a scoping review with the objective of identifying scientific evidence on the impact of drug use on the lives of elderly people, outlined in accordance with the JBI and PRISMA-ScR guidelines. The PCC strategy was used as a parameterized search strategy in the MEDLINE online databases through the PubMed, CINAHL, Scopus and Web of Science portals. The studies were exported to the Rayyan® online platform, where the studies that made up the sample were selected. These review stages were carried out in a double-blind manner. The information was extracted through a questionnaire prepared by the research team. The second stage consisted of a cross-sectional, observational, quantitative study, designed in accordance with the Strengthening the Reporting of Observational Studies in Epidemiology guidelines. Held in Basic Health Units and Psychosocial Care Centers for Alcohol and other Drugs in the municipalities of João Pessoa and Cabedelo, in Paraíba. The sample consisted of elderly people registered in the institutions that were the research settings, allocated into two groups, the first being made up of elderly people who abuse and use drugs and the second group made up of elderly people who occasionally use drugs. drugs from analysis using ASSIST 2.0. In addition, the MMSE, EDG-15, Family APGAR and a questionnaire containing personal information, social profile and health problems were applied. The data were analyzed using descriptive and inferential statistics with the aid of SPSS software. Results: The scoping review identified a relationship between the lack of family support and the propensity for excessive drug consumption, with consequences such as the development of physical illnesses and psychological disorders, and a greater predisposition to the risk of overdose and suicidal behavior. The observational crosssectional analytical study showed that drug use results in impairments in cognition, mood and family support. Several factors contribute to this scenario, including male sex, exposure to violent environments, adverse experiences in childhood and adolescence, dysfunctional family dynamics, presence of mental disorders and chronic non-communicable diseases. Conclusion: This study highlighted the repercussions of drug use on cognition, mood and family support, showing the damage to the physical, mental and social health of elderly people. Therefore, it is essential to develop health care strategies and public policies to promote specific health for elderly people who use drugs, aiming to promote healthy aging and prevent health problems in this vulnerable population.
