Influência da satisfação no controle da pressão arterial sistêmica na atenção primária: análise com modelagem de equações estruturais e indicadores compostos

Resumo: | The Primary Health Care through the Family Health Strategy, the user considers as an element of social transformation in the health-disease process, and from monitoring over time, may contribute in controlling the disease. In this sense to evaluate the satisfaction of the individual reflects the quality of health care services, from technical care, relationship to the user / service. In this context the study was to evaluate the satisfaction of hypertensive patients in the Family Health Strategy municipalities of João Pessoa / PB and Campina Grande / PB in the period from 2009 to 2011. This is an observational study of a population based retrospective cohort, respectively, with 308 hypertensive and 253 to Campina Grande Joao Pessoa registered in HiperDia in 2006/2007, using a validated questionnaire Paes 2008-2009. Sociodemographic variables were studied for degrees of satisfaction and the eight dimensions of Primary proposals by Starfield (Health of confirmed case; Access to diagnosis; Access to treatment; Adherence/bond; Services Catalogue; Coordination; Focus on family and Community orientation.). For each dimension was constructed a composite index, using a Likert scale (1 to 5) and the average of the responses. Nonparametric statistical tests were applied to determine the existence of significant differences between the degrees of satisfaction in time. The Structural Equation Modeling was used to observe the relationship between blood pressure control and "satisfaction." Predominant characteristics for hypertensive patients were age 60 years, female, white and brown for Campina Grande to João Pessoa; hipertensives lived with incomplete basic education. Results showed that hypertensive patients mostly showed elevated levels of "satisfaction" and a no pressure control for all periods above 50%. Dimension Adherence / bond was that obtained the highest degrees satisfaction and the lowest were found in the Guidance for Community dimension. Modeling to the models in "general satisfaction", "satisfaction PA controlled" and "satisfaction PA uncontrolled" confirmed that both indicate no influence each other. Methodology used to evaluate important aspects of "satisfaction" with the services has potential for this type of problem by extending the possibility of studies that contribute to the process of decision making by managers and professionals. Watching uncontrolled blood pressure and high levels of satisfaction has is a great paradox that a challenge to improve the quality of services and actions developed by the control plane of hypertension. |
