The general objective of the study is to “analyze care for users with chronic conditions in Primary Health Care, from the perspective of the care networks of the 4th Regional Health Coordination – RS”. As specific objectives, we have: to know the care flows of users with Type 2 Diabetes Mellitus and Systemic Arterial Hypertension, from the perspective of health care networks, considering the processes of care and self-care supported in the municipalities that integrate the 4th Regional Coordination of Health – RS and; to analyze the conceptions of professionals from a management group about the care process for users with type 2 Diabetes Mellitus and Systemic Arterial Hypertension in Primary Health Care in municipalities of the 4th Regional Health Coordination - RS. This is an exploratory-descriptive study, with a qualitative approach, developed in the municipalities of the 4th Regional Health Coordination of Santa Maria (CRS), linked to the State Health Department. This Coordination is organized in two health regions called Verdes Campos and Entre Rios, composed of 32 municipalities. The study included 21 professionals who are part of a reference group for Non-Communicable Diseases and Disorders (DANT), Health of the Elderly and Academia da Saúde. These professionals are predominantly linked to the public policy management sectors of the municipalities and have direct articulation with the management of DANT in the 4th CRS. Data collection was performed using a questionnaire composed of open questions, whose data were analyzed according to Thematic Content Analysis. The results showed that the care process for users in care networks is permeated by advances and challenges. Regarding advances, there is the existence of a specialized regional outpatient clinic, the role of Primary Care and the construction of clinical protocols and care flows, even if incipient. The challenges identified refer to the educational dimension in health, both for users and teams, the maintenance of qualification initiatives along the lines of the planning process and the strengthening of the steering group. Specifically in relation to PHC, the results suggest that this care process is characterized by attention to users with important risk factors and difficulties in adhering to care plans. Group approaches and the work of community health agents were shown to enhance this process. Some issues are still seen as necessary for the qualification of care, such as the resumption of educational groups in health, team meetings for case discussions and the strengthening of the active search. It is considered that user care needs to be qualified through health care and management strategies and, in this regard, the regional steering group needs to be strengthened as a permanent space for reflection and redesign of health care networks. This study contributes to reinforce the role of PHC in the care of chronic conditions, but, essentially, to make visible the existence of a guiding group, with regional scope, with the potential to share their experiences and advance in facing the challenges of the reality studied. |
