Elaboração de protocolo para assistência de enfermagem no parto de risco habitual : processo, expectativas e influências na prática

Resumo: ABSTRACT: This study aims to analyze the collaborative process of elaborating a care protocol for a Preterm/Childbirth/Puerperium unit of a university hospital in the city of Cuiabá-MT. This study is a Convergent Care Research developed in a Preterm/Childbirth/Puerperium unit of a university hospital. Data were collected through Convergence Groups, open interviews, and participant observation. The analysis of this set of information enabled to identify two organizational axes: 1) The use of the Convergent Care Research as a methodological tool for the elaboration of protocols - describing the process of elaboration of the care protocol to the usual childbirth risk, based on the methodological reference of the Convergent Care Research. The elaboration process was composed of nine steps: 1. Institutional Authorization; 2. Involvement and integration of participants; 3. Survey of unit problems; 4. Constant and flexible planning of the construction process; 5. Survey of scientific evidence; 6. Evaluation and organization of scientific evidence; 7. Convergence groups; 8. Textual elaboration of the protocol; 9. Institutional approval of the protocol; 2) The result of the protocol elaboration process. This axis shows the relevance of the use of Convergence Groups as a methodological strategy for the elaboration of protocols. The establishment of the group of obstetrical nurses, the movement to incorporate scientific evidence into care practice and the leveling of care practices stand out among the contributions of the elaboration process, since the protocol is a tool stimulating professional reflection for decision making, making care safer because it is based on the best available scientific evidence, ethical and humanized principles. Once the protocol is implemented, it will be considered as an instrument ensuring technical and legal support, professional autonomy and appreciation of the practices developed by nurses, besides to favoring the intended humanization of childbirth care. It is concluded that the process of creation and elaboration of the protocol contributed to the humanization of delivery and birth care since it influenced the rational use of health technologies and the insertion of good practices of childbirth and birth care. It became clear that obstetrical nurses are important agents in the process of demedicalization of delivery care since their practice is based on scientific evidence and the protagonism and autonomy of the parturient.
