Transferência de cuidados: o olhar e a prática dos profissionais de Enfermagem em um serviço de pronto atendimento

Resumo: The transfer of care between nursing professionals is related to the exchange of accurate and complete information about patients in the ER (Emergency Room). Some factors interfere with a quality transfer, such as an excessive number of patients with multiple comorbidities, limited past medical history and frequent interruptions. This study aimed to analyze the transfer of care performed by nursing professionals in the environment of an emergency care unit. A qualitative, descriptive study was carried out with 8 nurses and 3 nursing technicians. Data were collected in a large project, between September and December 2017, through consultation of documents and interviews with a semi-structured script and submitted to Content Analysis, focusing on secondary analysis of data from the nursing team that had not yet been analyzed. The results were organized into 3 categories: Transfer of care by the nursing team; Nursing work in an emergency care unit and the importance of communication in the transfer of care. The results show that the shift change is a moment that brings some communication difficulties due to work overload, a large number of patients with different levels of severity, sector dynamics, transfers within the ER, intra-hospital and inter-hospital (external transfers), place of shift change, little use of information recording instruments and interruptions. Professionals expressed the strengths and weaknesses of the process and the dynamics in the ER, as a form of care. The standardized patient transfer process between the intra and interhospital nursing teams should be structured to organize the transfer and records of patient information, reducing errors in care and data loss, in addition to avoiding adverse events and improve patient safety and job satisfaction.
