Análise em longo prazo do reganho de peso em pacientes obesos tratados com balão intragástrico

Resumo: Aims: Obesity is a global disease and its management includes pharmacological therapy, surgery and intragastric balloon (IGB). IGB has gained popularity recently, however, studies addressing long-term weight loss with IGB are scarce. This study aimed to assess the weight regain in a large cohort of patients treated with IGB, with a time span of two to five years after the removal of the device. Methods: Obese patients treated with intragastric balloon (IGB) for a 6-month period from February 2011 to December 2013 who underwent balloon removal between 2 and 5 years before the collection of data, were invited to participate in the study. The participants were interviewed and evaluated by independent researchers. Medical records were analyzed, along with body weight at IGB insertion and removal. Patients were stratified based on length of follow-up after IGB removal on the date of interview (2, 3, 4 and 5 years), and factors relevant to weight control as well as behavioral habits were analyzed and compared by using a logistic multivariate analysis. Results: A total of 224 patients were included in the study. Mean weight lost during treatment with IGB was 15,3±6,9 kg. After IGB removal, 67% of the individuals regained weight. The mean weight regain was 4.66±4.91 kg, 8.66±6.96 kg, 9.99±8.44 kg and 19.96±12.24 kg at follow-up on 2, 3, 4 and 5 years, respectively. Multivariate logistic analysis demonstrated that the following variables were significantly associated with weight regain: lack of psychological support during or after treatment (ODDS for 2y F-U: 1,13; p=0,02; IC95%: 0,55-1,89); lack of nutrition counseling (ODDS for 3y F-U: 3,36; p<0,01; IC95%: 1,42-7,94), and sedentary lifestyle (ODDS for 4y F-U: 3,86; p=0,03; IC95%: 1,13-12,67). Conclusion: Weight regain occurs commonly after intragastric balloon removal in up to twothirds of patients. Lack of psychological and nutritional follow up during and after treatment, as well as sedentary behavior are important risk factors for weight regain.
