Resumo: Primary Health Care evaluation presents itself as a challenge, given the complexity of the adopted model and its importance to the effectiveness of health care. The research has the National Program of Access and Quality Improvement in Primary Health Care as the main context and quality in health as the subject of analysis. The general objective is to analyse the similarities and divergences between the quality conceptions present in the official program documents and those constructed by users, and later reflecting about the consequences to Primary Health Care. Assumed as specific objectives: analyse official program documents to absorb and comprehend the meanings and internationalities present in the policy; analyse Primary Health Care users‘ opinions to comprehend the constructed conceptions related to health quality theme and correlate the meanings present in official documents and users‘ opinions. The software Analyse Lexicale par Contexte d´um Ensemble de Segments de Texte was used to investigate official policy documents through lexical and co-occurrences analysis, underlain by the Theory of Social Representations. A non-systematic observation was realized in two health units in Vitória and Vila Velha to analyse Health Care users‘ speeches, constructing a field diary and realizing open interviews. Lastly the conceptions present in Health Care policies are correlated to those constructed by users. The results indicate ambiguous and antagonistic quality conceptions in the program and a detachment from users, which see quality as guarantee of access to health care in heath units and to related services, with a solving and helpful approach in the contact with healthcare workers. This detachment compromises the effectiveness of actions. The research pointed out the necessity of investment in evaluative research towards the development of methodologies more adequate to Primary Health Care and instruments which allow to listen to the involved and interested parties, focusing the attention on the users.
