Resumo: The present essay has as objective the analysis of the elderly home care attention in Sao Paulo provided by it s Home Attendance Unity (UAD). The focus is to understand a service that, embedded into the health public policy, is oriented by the Home Attendance (AD) to the dependent and bedridden elderly. Objectives: study the historical implantation process of these services; identify the aspects related to the functioning and interface with different assistant levels; identify the guidelines that lay the foundation of the structure of the process; analyze the home attendance teams conceptions, its perspectives and refinement suggestions; identify the aspects of the team relation with the family and caretakers; and analyze the comprehension of the managers around the subject. Study and Methodology Orientation: it s a descriptive and qualitative study based on a critical and dialectical methodology-theorist approach. It was preceded from documental and bibliographical research around the subject. The research was realized in five UAD of the Regional Health Southeast Coordination (CRS) following three semi-structured scripts to the interviews: one script with a institutional profile, one oriented to the UAD professionals and one to the managers. Twelve professionals from the UAD were interviewed and three of them perform local or regional management levels. Conceptual references of the analysis: notion of care, pragmatics vulnerability and Networks of Health Attention (RAS). It was used the analysis technique of the content (MINAYO, 2004), considering the following axes: institutional profile of the UAD; profile of the professionals and managers; UAD everyday work routine; the teams home attendance conceptions; managers comprehension around home attendance. Results: the elderly compose the majority of the UAD demand. These services were implemented in the last decade and were motivated by: services rationalization and optimization of hospital services, in the economic scope; and, considering the social scope, the bedridden elderly demand of attendance. In the network that provides assistance, there are no references that can guarantee a integral care and attendance to the patient. The professionals reunite efforts to always provide qualitative attention although they present different conceptions, specially in its relation with the care giver, which they consider the key actor in this assistance modality. Final Considerations: The UAD justifies itself through the existing demand and nature of its actions. It requires effective insertion in the Network for Health Attention aiming to make clear the patient needs. Also it requires the insertion in health public politic deliberation as a possibility of withdraw the obscurity of the bedridden elderly and the daily challenges set up to its families
