Resumo: INTRODUCTION: The use of hearing aids (HA) is essential to restore the audibility of environmental and speech sounds, thus mitigating the impacts that hearing loss can cause. In Brazil, care for people with hearing impairments in the Unified Health System (SUS) began around the beginning of the 1990s. In 2004, the National Policy for Hearing Health Care was implemented, which provided significant evolution. in the field of Hearing Health. OBJECTIVE: To demographically characterize, over a period of 20 years, adult and elderly patients assisted at a specialized reference service for hearing impairment, considering the waiting time between the diagnosis and the fitting of the hearing aid, the number of follow-ups that each patient underwent and the time interval between them. METHOD: Observational, exploratory and descriptive study. Analysis of information from a database was performed. The variables analyzed were: age, sex, diagnosis, type of hearing loss, number of new cases, evasion, time between diagnosis and fitting of the hearing aid and the time between it and the first follow-up. Data analysis was performed according to the variables of interest with statistical analysis, initially performed using some summary measures, bar and line graphs. RESULTS: The research database consisted of 12,632 patients. Over the years, the number of new cases has increased. As for gender, there was a slight predominance of females. The arrangement of age groups did not remain the same over the years studied, but the predominance of elderly people remains over the years. The predominant type of hearing loss was sensorineural. There was a decrease in the time between the completion of the diagnosis and the first day of fitting the hearing aid over the years. It was seen that women return more frequently to follow-ups when compared to men. CONCLUSION: The analysis of the service users' characteristics over the 20 years of accreditation with the Ministry of Health demonstrates the gradual improvement and organization with the objective of better serving the population and service demand. There was an increase in vacancies offered for new cases from the year 2008 and a decrease in the time between diagnosis and first fitting of the hearing aid; users return at least once after receiving the hearing aid
