Associação entre o nível de resiliência e o estado clínico de pacientes renais crônicos em hemodiálise

Resumo: Introduction: Patients with chronic renal failure on dialysis suffer from a number of restrictions and physical and emotional losses. They have to have an ability to cope with such adversity using a series of psychological, social and emotional resources, called resilience. This concept can influence on how each patient responds to medical treatment and their clinical conditions facing it. The main goal of this study was to evaluate the association between the level of resilience and clinical status of patients with chronic renal failure on hemodialysis. Method: A cross-sectional, quantitative study, which measures and analyzes the distribution of levels of resilience displayed by patients on dialysis relating to their clinical status. The participants were 60 chronic renal failure patients of both sexes, adults (over 18), educated to at least the first grade without significant cognitive impairment, regardless of the underlying disease that led to dialysis and that this had started at least three months before the application of the study. The main tools used were the Resilience Scale, by Wagnild and Young (1993), the Mini Mental State Examination (MMSE, 1999) and Beck's Depression Inventory (2001). The clinical status of patients was measured by the average of the last three months prior to collection of three main parameters: Kt / V, hemoglobin and body mass index (BMI)Results: It was noticed a slight trend for patients with higher scores on the resilience index to have their Kt / V index closest to the considered optimal for a good dialysis process (greater than or equal to 1. 2 mg / dL), with r = 0, 19, and P = 0. 15, indicating a not significant association. When the relation between the level of resilience and the average of the last 3 months prior to the study of hemoglobin indeex was observed, an association even more discreet than the comparison with the index of Kt / V was found, with r = 0. 04 3 P = 0. 76. This indicates also a not significant association. Analyzing the relationship between the level of resiliency and body mass index (BMI) of patients evaluated, one sees a greater tendency than in comparison to the associations previously made, with r = 0. 27 and P = 0. 038. This means that possibly the patients who had body mass index within the level taken as healthy also had higher level of resilience. Conclusions: There was no statistically significant association between the level of resilience and clinical status of patients assessed, although the results indicate a slight tendency for it. Further studies are needed to deepen the concept of resilience in this field of study.
