Resumo: Introduction The Nursing Activities Score (NAS) is an index created in 2003. It is a result of changes of the Therapeutic Intervention Scoring System (TISS-28) which has as a goal to stimulate precisely the work load of nursing in the intensive Care Unit (ICU) and evaluate the needs of cares. It expresses the percentage of wasting time by a nursing professional in the close assistance on critical patient in the ICU during 24 hours. Objectives To valid NAS as a measure of nursing work load, compare it with the TISS-28 in the pediatric ICU and combine it to seriousness, morbidity and mortality. Methodology The study of the analytic observational prospective cohort, with a quantitative approach the study was made at a PICU of “Hospital da Criança Santo Antônio”, in Porto Alegre, in the general Pediatric ICU of type III. The sample was made by all children with age between 29 days and 12 years, who were hospitalized between August 25th 2008 and February 25th 2009. The collect was made by a tool adapted to TISS-28 and NAS, containing 27 therapeutic interventions and applied each 24 hours. It was made the nurses’ capacity (Kappa average of 0, 95). We used the criteria of Miranda and Conishi, it was considered that each point of TISS-28 was equalized to 10,6 minutes, and each point of NAS equalized 14,4 minutes. The points values of TISS-28 and NAS of each day of internment, the total, the first and the worst day, the average and discharge from hospital. The time of internment and reason of discharge were considered as variables of outcome. The study was approved by the Ethic Committee in research (ECR) of the Order of Santa Casa de Misericórdia de Porto Alegre (1887/08).Results 545 admissions compound the sample and 2951 observations were gotten. The admission TISS-28 ranged from 12-68 points, average of 33+12 and median of 30. The maximum TISS-28 ranged from 12-68 points, average of 34+13 and median of 32. NAS ranged from a score in admission of 40-130, average of 59+12 and median 59. The maximum NAS ranged from 40-130 points, average of 63+15 and median 61. About the score converted into minutes, it was realized that TISS-28 was superior to NAS according to all the observations (p<0,001). The observed mortality measured by PIM 2 was 6,2%. TISS-28 and NAS showed a good discrimination of mortality. Turn ROC of TISS-28 was 0,79 (IC95%-0,72/0,87) and of NAS was 0,84 (IC95%-0,75-0,92) in the admission and maximum score of TISS-28 was 0,87 (IC95%0,81-0,92), and NAS was 0,99 (IC95%0,99-1,00). The two indexes presented a significant, straight, positive, moderated co-relation with an R=0,564.Conclusions It was observed a good agreement between TISS-28 and NAS in the population with pediatric patients and, both present a good capacity of discrimination for mortality. It was found out the best point of cut for the risk of mortality among the scores; it refers to 16-hour-patient of nursing work per day.
