Motivação para a mudança de comportamento no jogo patológico: tradução, adaptação e validação da "University Rohde Island Change Assessment (URICA)"

Resumo: | The Transtheoretical Model of Behavior Change has been used in several dependent disorders as approach of understanding the process of change. This model is based on the premise that behavior change happens over a process in which people pass through various stages of motivation to change: Pre-contemplation, Contemplation, Preparation, Action and Maintenance. The University Rohde Island Change Assessment-URICA is a self-reported scale that was built to identify the motivational stages and readiness to change in various behaviors. This study aims to describe the process of translation, semantic equivalence and cultural adaptation of the URICA and identify the evidence of validation of this scale (reliability) between pathological gamblers. The questionnaire was translated from the original English version to Portuguese, using the process of translation-back translation (English-Portuguese- English), followed by an evaluation committee of specialists and approval of one of the authors of the scale. Later, it was studied the psychometric properties of the instrument in a sample of 69 subjects diagnosed with Pathological Gambling, according to diagnostic criteria of DSM-IV-TR. The subjects were evaluated in ambulatory care locals in Porto Alegre and São Paulo, and groups of Anonymous Gamblers, between 2006 and 2008. For the study of evidence of validation of URICA were analyzed indicators of internal consistency and convergent validity with the ruler of readiness (visual-analogue measure), as well as the instruments: SOGS (South Oaks Gambling Screen), BAI and BDI. There was need for vocabulary adjustments in some items, and it was decided by the inclusion of terms that refer to the "habit of gambling," referring to the behavior studied and the peculiarities of this sample. Descriptive analysis was carried out and there was a correlation between the judges of 75%, adequate reliability of URICA (\ = 0. 82) and significant correlation with the ruler of readiness in the pre-contemplation stage (r = -0305, p = 0. 11) and action (r = 0,301, p = 0. 12). The sample was divided into two groups, as the system of treatment: group from outpatient clinics specialized in treatment of addictions and participants of the group of Anonymous Gamblers in Porto Alegre. The outpatient group had significantly higher average scores than the Anonymous Gamblers (AG) both in the pre-contemplation stage (p = 0005) and in the maintenance stage (p = 0044). The abstinence period, the AG is associated with a time of abstinence superior than 180 days, while the outpatient group is associated with a period from 30 to 180 days in this sample. Considering the reports of specialists, it was assembled the final version of the Brazilian URICA-adaptation for pathological gambling that showed as easy application tool and with good semantic equivalence. Moreover, the URICA - Brazilian adjustment for pathological gambling had indicated useful and reliable for evaluating the motivation to change, considering the motivational stages and readiness to change among pathological gamblers. |
