Resumo: Vaccines are immunobiological compounds by one or more immunizing agents, and their main feature is the reduction of morbidity and mortality from preventable diseases. The need for a systematic assessment of vaccine losses is an important step for health management across the country. Still, the diagnosis of the main causes of immunobiological losses in the Basic Health Units (UBS), can contribute to the management allowing to organize the system, thus avoiding the waste of public resources. This study aimed to carry out an analysis of the main causes of losses of immunobiologicals at the UBS in the city of Guarulhos - SP in the year 2019, and to measure the values associated with these losses. This is a descriptive, retrospective, quantitative study, carried out based on reports of specific movement, technical loss, and physical loss extracted from SIPNI, and the invoices received with immunobiologicals. An annual fee was charged for applied doses, physical losses, technical losses and non-categorized doses. It was also calculated as total rates of losses, prevalence and costs per immunobiological agent. Of the doses made available at UBS, 60.63% were applied, and the total loss was 39.37%, with the prevalence of technical loss being 20.42%, and the non-categorized doses of 11.76%. The immunobiologicals with the highest percentage of total loss in doses were Triple Viral (19.99%), Yellow Fever (18.19%) and Double Adult (11.20%), and those with the highest percentage of total loss in values were Triple Viral (16.62%), chickenpox (12.61%) and Pneumo 10 (9.67%). The results showed that a rate of vaccine losses at UBS in 2019 was 39.37%, obtaining a higher prevalence of technical losses. The research led to the idealization of 10 proposals aimed at reducing the losses of immunobiologicals in the city of Guarulhos, which will be carried out by the bodies of the Health Secretariat, namely: Department of Health Surveillance and Technical Division of Epidemiology and Disease Control.
