Oral Lichen Planus (OLP) is a chronic mucocutaneous inflammatory autoimmune disease. It is may be asymptomatic or have severe pain in case of atrophic / erosive lesions. The standard treatment consists of using topical and systemic corticosteroids, which are associated with clinical improvement although there may be side effects or a lack of response to the therapy. The Photobiomodulation (FBM) has been shown to be effective in the treatment of inflammatory diseases, promoting analgesic effect and accelerating the repair process. There are no studies in the literature that evaluated the effects of FBM on the modulation of inflammatory cytokines in OLP. Thus, this study aims to evaluate the concentration of IL-1β, IL-4, IL-6, IL-10, IL-17A e TNF-α cytokines in the serum of patients with OLP, treated with FBM, when compared to conventional treatment, as well as the anxiety and depression levels, before and after the treatment. Thirty four patients diagnosed with OLP were randomized into two groups, FBM and control (0.05% clobetasol propionate). The irradiation was performed twice a week with low power InGaAIP diode laser (680 ± 20nm, power 100mW, energy density 177J / cm2, 5 seconds, 0.5J total energy per point) for 4 consecutive weeks Peripheral blood was collected at baseline (D0) and at the end of treatment (D30), and the cytokines concentrations were evaluated by ELISA test. The Hospital Anxiety and Depression Scale (HADS) questionnaire was applied to patients at D0, D7, D14, D21, D30, D60, D90 and D120 to assess levels of anxiety and depression. IL-17A cytokine was the most concentrated in OLP patients at baseline. There was no significant difference in the serological levels of IL-1β, IL-4, IL-10, IL-17A and TNF-α before and after treatment, both in FBM and Control group. There was difference only in IL-6 after treatment in corticoid group. There was no correlation between cytokine concentrations and pain or clinical score. There was also no statistical difference in anxiety and depression levels, before and after treatment in OLP patients. |
