Summary: The widespread use of fluoride has interfered with the progression of carious lesions, with situations being observed where the structure of the dental enamel is sound or practically intact and the lesion progresses slowly in the dentin. These lesions are classified with code 4 by the International System for the Detection and Assessment of Caries (ICDAS) (International Caries Detection and Assessment System) and are difficult to decide on the best clinical intervention. This review aimed to review the possible ways of intervening in dentin caries lesions, classified with the ICDAS 4 code, on minimal intervention techniques, in order to verify which treatment is more conservative for use in clinical practice. A literature review was carried out using the Pubmed, Scielo and Lilacs databases, using the keywords: “dental caries”, “dentin”, “removal”, both in Portuguese and in English. The treatment of caries lesions by the selective removal technique proved to be effective and indicated whenever the extent of the lesions reach more than half of the same, as they preserve the tooth structure and prevent exposure of the pulp. The sealing of the lesions is effective and has shown to be indicated, being more favorable in lesions that reach up to the middle of the dentin. It was concluded that both treatments were efficient and should be indicated based on clinical criteria and the best scientific evidence.
