Intervenção clínica para lesões de cárie em dentina de dentes permanentes posteriores: revisão de literatura

Shrnutí: | The widespread use of fluoride has interfered with the progression of carious lesions, with situations being observed where the structure of the dental enamel is sound or practically intact and the lesion progresses slowly in the dentin. These lesions are classified with code 4 by the International System for the Detection and Assessment of Caries (ICDAS) (International Caries Detection and Assessment System) and are difficult to decide on the best clinical intervention. This review aimed to review the possible ways of intervening in dentin caries lesions, classified with the ICDAS 4 code, on minimal intervention techniques, in order to verify which treatment is more conservative for use in clinical practice. A literature review was carried out using the Pubmed, Scielo and Lilacs databases, using the keywords: “dental caries”, “dentin”, “removal”, both in Portuguese and in English. The treatment of caries lesions by the selective removal technique proved to be effective and indicated whenever the extent of the lesions reach more than half of the same, as they preserve the tooth structure and prevent exposure of the pulp. The sealing of the lesions is effective and has shown to be indicated, being more favorable in lesions that reach up to the middle of the dentin. It was concluded that both treatments were efficient and should be indicated based on clinical criteria and the best scientific evidence. |

Essas lesões são classificadas com o código 4 pelo Sistema Internacional para a Detecção e Avaliação de Cáries (ICDAS) (do inglês, International Caries Detection and Assessment System) e são de difícil decisão sobre qual a melhor intervenção clínica. A presente revisão se propôs a revisar as possíveis formas de intervir em lesões de cárie em dentina, classificadas com o código ICDAS 4, sobre técnicas de mínima intervenção, a fim de verificar qual o tratamento é mais conservador para utilização na prática clínica. Foi realizada uma revisão da literatura, através das bases de dados Pubmed, Scielo e Lilacs, utilizando as palavras-chave: “cárie dentárias”, “dentina”, “remoção”, tanto em Português, quanto em Inglês. O tratamento das lesões de cárie pela técnica de remoção seletiva se mostrou efetivo e indicado sempre que a extensão das lesões atingem mais que a metade da mesma, pois preservam a estrutura dental e previnem a exposição da polpa. O selamento das lesões é efetivo e demonstra ter indicação, sendo mais favorável em lesões que atinjam até a metade da dentina.
