Minimally invasive TMJ surgery: arthrocentesis x arthroscopy

Summary: Arthrocentesis and arthroscopy are two minimally invasive surgical procedures designed to treat internal TMJ disorders. This study aimed to compare the pre- and postoperative clinical parameters between the surgical techniques of arthrocentesis and arthroscopy, through a literature review. This is an integrative literature review study, carried out through national and international articles, published in the PubMed, SciELO and CAPES/MEC databases, in the period between 2010 and 2021. Four descriptors were used in Portuguese and English, taken from DeCS. The search for articles was based on the descriptors. After applying the inclusion and exclusion criteria, four articles formed the final sample. Both techniques have excellent success rates in terms of long-term pain relief, immediate improvement in the postoperative period, in mouth opening, as well as in the position of the articular disc. Occurring in low complication rates, which characterizes them as safe procedures. The two techniques demonstrate similarities in success rate with effective results. The choice of the surgical modality should be attributed to the dentist along with the patient, taking into account the evaluation of clinical exams and imaging exams in association with the socioeconomic condition of the patient.

Este estudio tuvo como objetivo comparar los parámetros clínicos pre y postoperatorios entre las técnicas quirúrgicas de artrocentesis y artroscopia, a través de una revisión de la literatura. Se trata de un estudio de revisión integradora de la literatura, realizado a través de artículos nacionales e internacionales, publicados en las bases de datos PubMed, SciELO y CAPES / MEC, en el período comprendido entre 2010 y 2021. Se utilizaron cuatro descriptores en portugués e inglés, tomados de DeCS. La búsqueda de artículos se basó en los descriptores. Después de aplicar los criterios de inclusión y exclusión, cuatro artículos conformaron la muestra final. Ambas técnicas tienen excelentes tasas de éxito en términos de alivio del dolor a largo plazo, mejoría inmediata en el postoperatorio, en la apertura de la boca, así como en la posición del disco articular. Ocurriendo en bajas tasas de complicaciones, lo que los caracteriza como procedimientos seguros. Las dos técnicas demuestran similitudes en la tasa de éxito con resultados efectivos. La elección de la modalidad quirúrgica debe atribuirse al odontólogo junto con el paciente, teniendo en cuenta la evaluación de exámenes clínicos y exámenes de imagen en asociación con la condición socioeconómica del paciente.A artrocentese e artroscopia são dois procedimentos cirúrgicos minimamente invasivos designados ao tratamento dos distúrbios internos da ATM. Este estudo objetivou comparar os parâmetros clínicos pré e pós-operatórios entre as técnicas cirúrgicas da artrocentese e da artroscopia, por meio de uma revisão bibliográfica. Trata-se de um estudo de revisão de literatura integrativa, realizado através de artigos nacionais e internacionais, publicados nas bases de dados PubMed, SciELO e CAPES/MEC, no período entre 2010 e 2021. Foram utilizados quatro descritores nos idiomas português e inglês, retirados do DeCS. A busca dos artigos se deu a partir dos descritores. Após a aplicação dos critérios de inclusão e exclusão, quatro artigos formaram a amostra final. Ambas as técnicas apresentam excelentes percentuais de sucesso no que se refere ao alívio da dor a longo prazo, uma melhora imediata no pós-operatório, na abertura bucal, bem como, na posição do disco articular. Ocorrendo em baixos índices de complicações, o que as caracterizam como procedimentos seguros. As duas técnicas demonstram semelhanças na taxa de sucesso com resultados efetivos.
