Prevetion of falls in the elderly in primary care: a systematic review

Summary: | This study aims to gather and analyze evidence on physical therapy actions and interventions applied in primary care in order to prevent the risk of falls in the elderly. This is a systematic review, filed with PROSPERO, carried out in the BVS, PEDro and Scielo databases. The Guidleline Preferred Reporting Items for Systematic Reviews and Meta-Analyses –The PRISMA Statement was used. The research question was: What are the actions and interventions of physical therapy that are applied in primary care to prevent the risk os falls in the elderly? A total of 85 studies were found and, after analysis, seven studies were included in this work. It was possible to verify the diversity of occurrence of studies in their continent of publication namely: South America (n=3), Europe (n=2), Oceania (n=1) and Asia (n=1). As for the level of evidence, the majority (n=4) was level 2 and only three (n=3) had level of evidence 3. There was a consensus that the applicability of preventive measures in primary health care was effective in reducing risk of falls in the elderly, but there was no consensus on the use of a specif protocol. The implementation of training programs and specific exercises performed in primary health care has a significant impact on improving balance, muscle strength, proprioception, functional capacity and quality of life of the elderly assuming an important role in the prevention of falls. |

Se trata de una revisión sistemática, archivada en PROSPERO, realizada en las bases de datos BVS, PEDro y Scielo. Se utilizaron los elementos de informe preferidos de la guía para reviciones sistemáticas y metanálisis: la declaración PRISMA. La pergunta de investigación fue: Cuáles son las acciones e intervenciones de fisioterapia que se aplican en la atención primaria para prevenir el riesgo de caídas en los ancianos? Se encontraron um total de 85 estudios y, después del análisis, siete estúdios fueron incluídos en este trabajo. Fue posible verificar la diversidad de ocurrencia de los estúdios en su continente de publicación, a saber: América del Sur (n=3), Europa (n=2), Oceanía (n=1) y Asia (n=1). En cuanto al nível de evidencia, la mayoría (n=4) fue de nível 2 y solo tres (n=3) tuvieron nível de evidencia 3. Hubo consenso en que la aplicabilidad de las medidas preventivas en la atención primaria de salud fue efectiva en la reducción del riesgo de caídas em ancianos, pero no hubo consenso sobre el uso de um protocolo específico. La implementación de programas de entrenamiento y ejercicios específicos realizados en atención primaria de salud tiene um impacto significativo en la mejora del equilíbrio, la fuerza muscular, la propiocepción, la capacidad funcional y la calidad de vida de las personas mayores, asumiendo un papel importante en la prevención de caídas.Este estudo tem por objetivo, reunir e analisar evidências sobre as ações da fisioterapia aplicadas na atenção primária com finalidade finalidade de prevenir o risco de queda em idosos. Trata-se uma revisão sistemática, protocolada no PROSPERO, realizada nas bases de dados BVS, PEDro e Scielo. Utilizou-se o Guideline Preferred Reporting Items for Systematic Reviewsand Meta-Analyses - The PRISMA Statement. A pergunta da pesquisa foi: quais são as ações e intervenções da fisioterapia que são aplicadas na atenção primária para prevenir o risco de quedas em idosos? Foram encontrados um total de 85 estudos, sendo que, após a análise, foram incluídos neste trabalho sete estudos. Foi possível verificar a diversidade de ocorrência dos estudos no seu continente de publicação, sendo eles: América do Sul (n=3), Europa (n=2), Oceania (n=1) e Ásia (n=1). Quanto ao nível de evidência, a maioria (n=4) foi de nível 2 e apenas três (n=3) com nível de evidência 3. Houve um consenso de que a aplicabilidade de medidas preventivas na atenção primária à saúde foi eficaz na redução de risco de quedas de idosos, porém não houve consenso na utilização de protocolo específico.
