Analysis of the efficacy of lithium in the prevention of suicide in patients with mood disorders: an integrative review

Summary: Background: Mood disorders refer to those whose main symptom is a change in mood or affect, altering animosity and the way of feeling, thinking and behaving. Since the 1970s, a number of primary studies have pointed to lithium's effectiveness in reducing the risk of suicide. Objectives: To carry out an integrative review of the literature on the efficacy of lithium in preventing suicide risk in patients with mood disorders. Methodology: Integrative review, which used the descriptors “lithium”, “prevention” and “suicide” in the LILACS, SciELO and PUBMED databases. Inclusion criteria were articles published in English, of the randomized clinical trial type, between 2000 and 2022, indexed in the chosen databases and available for full reading. The research was carried out according to the PRISMA flowchart for integrative and systematic reviews. Six articles were selected for review. Results and Discussion: Three studies showed that lithium did not reduce suicide attempts in patients with bipolar affective disorder (BD) or major depression. Differences in lithium doses, sociodemographic conditions, type of control group and type of mood disorder may cause the results to be discrepant when compared with each other. Conclusion: Although lithium is used to prevent suicidal behavior in patients with mood disorders, the studies analyzed showed divergent results regarding its effectiveness in this group.

Desde la década de 1970, varios estudios primarios han señalado la eficacia del litio para reducir el riesgo de suicidio. Objetivos: Realizar una revisión integradora de la literatura sobre la eficacia del litio en la prevención del riesgo de suicidio en pacientes con trastornos del estado de ánimo. Metodología: Revisión integrativa, que utilizó los descriptores “litio”, “prevención” y “suicidio” en las bases de datos LILACS, SciELO y PUBMED. Los criterios de inclusión fueron artículos publicados en inglés, del tipo ensayo clínico aleatorizado, entre 2000 y 2022, indexados en las bases de datos elegidas y disponibles para lectura completa. La investigación se llevó a cabo de acuerdo con el diagrama de flujo PRISMA para revisiones integradoras y sistemáticas. Se seleccionaron seis artículos para su revisión. Resultados y Discusión: Tres estudios mostraron que el litio no redujo los intentos de suicidio en pacientes con trastorno afectivo bipolar (TB) o depresión mayor. Las diferencias en las dosis de litio, las condiciones sociodemográficas, el tipo de grupo de control y el tipo de trastorno del estado de ánimo pueden hacer que los resultados sean discrepantes cuando se comparan entre sí. Conclusión: Si bien el litio se utiliza para prevenir la conducta suicida en pacientes con trastornos del estado de ánimo, los estudios analizados mostraron resultados divergentes en cuanto a su efectividad en este grupo.Introdução: Os transtornos do humor se referem àqueles cujo o sintoma principal é a alteração do humor ou do afeto, alterando a animosidade e o jeito de sentir, pensar e se comportar. Desde a década de 1970, uma série de estudos primários apontam para a eficácia do lítio na redução do risco de suicídio. Objetivos: Realizar uma revisão integrativa da literatura sobre a eficácia do lítio na prevenção do risco de suicídio em pacientes com transtornos do humor. Metodologia: Revisão integrativa, que utilizou os descritores “lítio”, “prevenção” e “suicídio” nas bases de dados LILACS, SciELO e PUBMED. Os critérios de inclusão foram artigos publicados em inglês, do tipo ensaio clínico randomizado, entre 2000 e 2022, indexados nas bases de dados escolhidas e disponíveis para leitura na íntegra. A pesquisa foi realizada de acordo com o fluxograma PRISMA para revisões integrativa e sistemática. Foram selecionados 6 artigos para a revisão. Resultados e discussão: Três estudos evidenciaram que o lítio não reduziu as tentativas de suicídio em pacientes com transtorno afetivo bipolar (TAB) ou com depressão maior. A diferença nas doses de lítio, nas condições sociodemográficas, no tipo de grupo controle e no tipo de transtorno de humor podem fazer com que os resultados sejam discrepantes quando comparados uns com os outros.
