Tuberculosis diagnosis accuracy by quick test – GENEXPERT/MTB: integrative review

Summary: Tuberculosis is a communicable disease caused by Mycobacterium tuberculosis. It mainly affects the lungs, but can infect other organs. It presents symptoms such as cough, which can be productive or not, afternoon fever, night sweats and chest pain, asthenia, anorexia and weight loss. Diagnosis can be made through several techniques, with sputum being the material most colonized by Mycobacterium, therefore, the most used. Polymerase chain reaction (PCR) based methodologies are fast and sensitive, gradually becoming the preferred diagnosis of tuberculosis. Method capable of identifying small amounts of the pathogen in a wide variety of clinical material. The GeneXpert/MTB test has greater sensitivity, specificity, simultaneous identification of deoxyribonucleic acid (DNA) of the Mycobacterium tuberculosis complex, capacity to detect mutations resistant to Rifampicin and the possibility of detecting new cases in less time. The study aims to describe the studies that evaluated the accuracy of tuberculosis diagnosis by the GeneXpert/MTB rapid test. In an integrative review, 13 studies were selected that met the proposed objective. Many studies defend numerous advantages such as speed, sensitivity and acceptable specificity, which stand out in comparison with other diagnostic methods.

Afecta principalmente a los pulmones, pero puede infectar otros órganos. Presenta síntomas como tos, que puede ser productiva o no, fiebre vespertina, sudores nocturnos y dolor torácico, astenia, anorexia y pérdida de peso. El diagnóstico se puede realizar mediante diversas técnicas, siendo el esputo el material más colonizado por Mycobacterium, por tanto, el más utilizado. Las metodologías basadas en la reacción en cadena de la polimerasa (PCR) son rápidas y sensibles, convirtiéndose gradualmente en el diagnóstico preferido de tuberculosis. Método capaz de identificar pequeñas cantidades del patógeno en una amplia variedad de material clínico. La prueba GeneXpert / MTB tiene mayor sensibilidad, especificidad, identificación simultánea del ácido desoxirribonucleico (ADN) del complejo Mycobacterium tuberculosis, capacidad para detectar mutaciones resistentes a la rifampicina y la posibilidad de detectar nuevos casos en menos tiempo. El estudio tiene como objetivo describir los estudios que evaluaron la precisión del diagnóstico de tuberculosis mediante la prueba rápida GeneXpert / MTB, en una revisión integradora se seleccionaron 13 estudios que cumplieron con el objetivo propuesto. Numerosos estudios defienden numerosas ventajas como la rapidez, la sensibilidad y la especificidad aceptable, que destacan frente a otros métodos diagnósticos.  A tuberculose é uma doença transmissível causada pelo Mycobacterium tuberculosis. Acomete principalmente nos pulmões, mas pode infectar outros órgãos. Apresenta sintomas como tosse, que pode ser produtiva ou não, febre vespertina, sudorese noturna e dor torácica, astenia, anorexia e emagrecimento. O diagnóstico pode ser através de várias técnicas, sendo o escarro o material mais colonizado pelo Mycobacterium, portanto o mais utilizado. As metodologias baseadas na reação em cadeia da polimerase (PCR) são rápidas e sensíveis, gradativamente têm se tornado o diagnóstico da tuberculose preferencial. Método capaz de identificar pequenas quantidades do patógeno em ampla diversidade de material clínico. O teste GeneXpert/MTB apresenta maior sensibilidade, especificidade, identificação simultânea do ácido desoxirribonucleico (DNA) do complexo Mycobacterium tuberculosis, capacidade de detectar mutações resistentes à Rifampicina e a possibilidade de detecção de casos novos, em menor tempo. O estudo tem o objetivo descrever os estudos que avaliaram a acurácia do diagnóstico da tuberculose pelo teste rápido GeneXpert/MTB, em uma revisão integrativa, foram selecionados 13 estudos que atendiam o objetivo proposto.
