Summary: Neonatal mortality remains one of the greatest challenges facing public health. Brazil has achieved the goal of reducing child mortality by two thirds, but still has high rates of neonatal deaths. The concept of neonatal near miss (NMN) has been shown to be a useful tool to evaluate the quality of neonatal health care and to point out ways to reduce mortality in the first month of life. A cross-sectional study was carried out nested in a cohort study at a high risk maternity hospital in Aracaju, Sergipe, Brazil, in which cases of NMN were identified and data on gestation and delivery were collected through interviews and medical records. A subsequent online consultation was made to identify late neonatal deaths among NMN cases. A total of 5,408 records were analyzed and 235 cases of NMN were identified, of which 12 died in the late neonatal period. The results point to shortcomings in maternal and child healthcare and to the need for improvements in public policies aimed at pregnant women and their newborns.

O Brasil atingiu a meta de reduzir em dois terços a mortalidade infantil, porém ainda apresenta elevadas taxas de óbitos neonatais. O conceito de near miss neonatal (NMN) vem se mostrando como uma útil ferramenta para avaliar a qualidade da assistência à saúde neonatal e apontar caminhos para diminuir a mortalidade no primeiro mês de vida. O presente estudo tem como objetivo descrever as condições materno-infantis dos RNs considerados near miss neonatal que sobreviveram à primeira semana, mas faleceram ainda no primeiro mês de vida. Foi realizado um estudo transversal aninhado a um estudo de coorte em uma maternidade de alto risco em Aracaju, Sergipe, Brasil, em que foram identificados casos de NMN e dados referentes à gestação e ao parto foram colhidos através de entrevista e consulta aos prontuários. Uma posterior consulta online foi feita para identificação dos óbitos neonatais tardios entre os casos de NMN. Foram analisados 5.408 prontuários e identificados 235 casos de NMN, sendo que 12 foram a óbito no período neonatal tardio. Os resultados apontam para deficiências na atenção à saúde materno-infantil e para a necessidade de melhorias nas políticas públicas voltadas para gestantes e seus recém-nascidos.
