Processo de adesão ao tratamento da tuberculose no contexto da Atenção Primária à Saúde: teoria fundamentada

Resumo: Adherence to tuberculosis treatment is one of the main challenges for the control of the disease. The strategies adopted so far are not considered effective enough to guarantee therapeutic success in all realities. The absence of a theory that adequately explains this adherence makes progress in research and development of this action difficult. In this context, this research aimed to understand how the process of adherence to tuberculosis treatment occurs in Primary Health Care. This is a qualitative study, which followed the Straussian strand of Grounded Theory as a methodological framework. Twenty people participated in the study, distributed into three sample groups: 1 – eleven people who underwent complete tuberculosis treatment between 2018 and 2019; 2 – seven professionals from Basic Health Units in the city of Cajazeiras-PB, being three doctors, three nurses and one Community Health Agent; 3 – two relatives of participants in the first group. Data were collected through interviews, from 2019 to 2021, concurrently with the analysis. Open, axial and integration coding procedures, construction of diagrams and memos, as well as analysis for context and process were considered. A theoretical model was developed, considering the paradigm, an analytical tool that helps to classify concepts and establish relationships. The NVivo® software (version 12 as a technological support for data organization) was used. The study was approved by CEP/UFRN, under opinion nº 3.246,634. The three components were named: conditions – feeling threatened with recovery; actions-interactions – carrying out the tuberculosis treatment and consequences – restoring health. The categories, subcategories and their respective concepts were interconnected and represented by the theoretical model, whose central category was “Feeling threatened, undergoing tuberculosis treatment and recovering health: transmuting suffering into a new life”. The main target of the people who adhered to the treatment was the recovery and solution of health threats. For this, they needed to modify the situations experienced, from the transformation of circumstances that generate suffering to those that promote a new life. Based on the constructed theoretical model, the contextual and procedural nature of adherence to tuberculosis treatment was observed, which permeates the experiences of the affected individual from before diagnosis to completion of treatment. The interaction with the data, theoretical and methodological procedures also showed that adherence to tuberculosis treatment is linked to aspects of life that existed prior to the illness, in the individual, family, social and labor spheres. It is considered that the results support the thesis: that people with tuberculosis act in relation to their care, anchored in what it represents for them. Therefore, the beginning of treatment is marked by physical, psychological and programmatic threats, which leads to questions about recovery. The second moment is marked by the beginning of the recovery process, leading to a process of resignification of life and awareness of being transmuted from suffering to a new life when undergoing the treatment.

As estratégias até então adotadas não são consideradas efetivas o suficiente para garantir sucesso terapêutico em todas as realidades. A ausência de uma teoria que explique essa adesão adequadamente dificulta o progresso na pesquisa e no desenvolvimento dessa ação. Nesse contexto, essa pesquisa teve como objetivo compreender como ocorre o processo de adesão ao tratamento da tuberculose na Atenção Primária à Saúde. Trata-se de um estudo qualitativo, que seguiu como referencial metodológico a vertente Straussiana da Teoria Fundamentada nos Dados. Participaram do estudo 20 pessoas distribuídas em três grupos amostrais: 1 – onze pessoas que realizaram o tratamento completo da tuberculose entre os anos de 2018 e 2019; 2 – sete profissionais de Unidades Básicas de Saúde do município de Cajazeiras-PB, sendo três médicos, três enfermeiras e uma Agente Comunitária de Saúde; 3 – dois familiares de participantes do primeiro grupo. Os dados foram coletados por meio de entrevistas, no período de 2019 a 2021, de forma concomitante à análise. Foram considerados procedimentos de codificações aberta, axial e integração, construção de diagramas e memorandos, como também análise para contexto e processo. Elaborou-se um modelo teórico, ao considerar o paradigma, uma ferramenta analítica que ajuda a classificar conceitos e estabelecer relações. Foi utilizado o software NVivo® (versão 12 como suporte tecnológico para organização dos dados. O estudo foi aprovado pelo CEP/UFRN, sob parecer de nº 3.246.634. Nomeou-se os três componentes: condições – sentindo-se com a recuperação ameaçada; açõesinterações – realizando o tratamento da tuberculose e consequências – recuperando a saúde. As categorias, as subcategorias e seus respectivos conceitos, foram interligados e representados pelo modelo teórico, cuja categoria central foi “Sentindo-se ameaçado, realizando o tratamento da tuberculose e recuperando a saúde: transmutando do sofrimento para uma vida nova”. O alvo central das pessoas que aderiram ao tratamento era a recuperação e solução de ameaças à saúde. Para isso, precisaram modificar as situações vivenciadas, a partir da transformação de circunstâncias geradoras de sofrimento para as promotoras de uma vida nova. A partir do modelo teórico construído, foi constatado o caráter contextual e processual da adesão ao tratamento da tuberculose, o qual perpassa por vivências do indivíduo acometido desde antes do diagnóstico até a finalização do tratamento. A interação com os dados, os procedimentos teóricos e metodológicos mostraram ainda que a adesão ao tratamento da tuberculose está atrelada aos aspectos da vida que se tinha anteriormente ao adoecimento, nos âmbitos individual, familiar, social e laboral. Considera-se que os resultados sustentam a tese: de que as pessoas com tuberculose agem em relação ao seu cuidado, ancoradas no que este representa para si. Para tanto, o início do tratamento é marcado por ameaças físicas, psicológicas e programáticas, o que leva a questionamentos sobre a recuperação.
