Summary: To develop a tool to assess the diabetic foot in Primary Health Care in a health unit in the east of Maranhão. This is a methodological study, carried out in the Family Health Strategy (ESF), developed in three stages: 1) Construction based on the literature review; 2) Application of the instrument for the pre-test with the target audience; 3) Application of the content by post-test professionals. In the pre-test phase, 53 patients were identified, the first version of the instrument consisted of 8 items, at the end of the pre-test, the identification items, assessment of risk factors, risk stratification and self-care capacity, the last stage of construction and validation of this instrument was the application by the higher education professionals of the ESF Pirajá. The use of a technology in health services contributes to the adequacy and improvement of the assistance provided.

Se trata de un estudio metodológico, realizado en la Estrategia Salud de la Familia (ESF), desarrollado en tres etapas: 1) Construcción a partir de la revisión de la literatura; 2) Aplicación del instrumento para el pre-test con el público objetivo; 3) Aplicación del contenido por parte de profesionales post-test. En la fase de pretest se identificaron 53 pacientes, la primera versión del instrumento constó de 8 ítems, al finalizar el pretest, los ítems de identificación, valoración de factores de riesgo, estratificación de riesgo y capacidad de autocuidado , la última etapa de construcción y validación de este instrumento fue la aplicación por parte de los profesionales de la educación superior de la ESF Pirajá. El uso de una tecnología en los servicios de salud contribuye a la adecuación y mejora de la asistencia prestada.Desenvolver uma ferramenta de avaliação do pé diabético na Atenção Primaria à Saúde em uma unidade de saúde do leste maranhense. Na fase pré-teste, identificou-se 53 pacientes; a primeira versão do instrumento foi constituída de 8 itens, e ao final do pré-teste, adaptou-se os itens de identificação, avaliação dos fatores de risco, estratificação de risco e capacidade de autocuidado; a última etapa de construção e validação desse instrumento foi a aplicação pelos profissionais de nível superior da ESF Pirajá.
