Trajectories in the care of homeless individuals with tuberculosis diagnosis: an integrative review

Summary: This study reviewed research from 2013 to 2023 on the healthcare trajectories of tuberculosis (TB) among people experiencing homelessness (PEH), analyzing its impact on treatment. Out of 87 studies found, 10 were selected after screening, all published between 2015 and 2023. These studies emphasized the care practices and challenges faced by PEH, highlighting social invisibility, structural barriers, and stigma. Most used qualitative methodologies, exploring the services provided by the Street Clinic teams (CnaR) in various regions of Brazil. High alcohol and drug use among PEH was a complicating factor in TB treatment. Factors such as lack of stable housing, food insecurity, and prejudice also negatively influenced treatment adherence. However, integrated approaches that addressed the social and health needs of PEH proved effective. The CnaR teams played a crucial role in offering comprehensive and personalized care, strengthening the bond of trust. Treatment adherence improved with continuous and tailored interventions. The research underscores the need for public policies that integrate medical care and support, aligned with the specific demands of PEH.

De los 87 estudios encontrados, 10 fueron seleccionados tras la evaluación, todos publicados entre 2015 y 2023. Estos estudios enfatizaron las prácticas de cuidado y los desafíos enfrentados por las PVS, destacando la invisibilidad social, las barreras estructurales y el estigma. La mayoría utilizó metodologías cualitativas, explorando la atención brindada por los equipos del Consultorio en la Calle (CeC) en diversas regiones de Brasil. El elevado consumo de alcohol y drogas entre las PVS fue un factor complicador en el tratamiento de la TB. Factores como la falta de vivienda estable, la inseguridad alimentaria y los prejuicios también influyeron negativamente en la adherencia al tratamiento. Sin embargo, enfoques integrados, que atendieron las necesidades sociales y de salud de las PVS, demostraron ser efectivos. Los equipos del CeC desempeñaron un papel crucial al ofrecer atención integral y personalizada, fortaleciendo el vínculo de confianza. La adherencia al tratamiento mejoró con intervenciones continuas y adaptadas. La investigación resalta la necesidad de políticas públicas que integren cuidados médicos y apoyo, alineadas con las demandas específicas de las PVS.Este estudo revisou pesquisas de 2013 a 2023 sobre trajetórias assistenciais de tuberculose (TB) entre pessoas em situação de rua (PVSR), analisando seu impacto no tratamento. Dos 87 estudos encontrados, 10 foram selecionados após triagem, todos publicados entre 2015 e 2023. Esses estudos enfatizaram as práticas de cuidado e os desafios enfrentados pelas PVSR, destacando a invisibilidade social, barreiras estruturais e o estigma. A maioria utilizou metodologias qualitativas, explorando o atendimento pelas equipes do Consultório na Rua (CnaR) em diversas regiões do Brasil. O uso elevado de álcool e drogas entre as PVSR foi um fator complicador no tratamento da TB. Fatores como a falta de moradia estável, insegurança alimentar e preconceitos também influenciaram negativamente a adesão ao tratamento. No entanto, abordagens integradas, que atenderam às necessidades sociais e de saúde das PVSR, mostraram-se eficazes. As equipes do CnaR desempenharam um papel crucial ao oferecer cuidado integral e personalizado, fortalecendo o vínculo de confiança. A adesão ao tratamento melhorou com intervenções contínuas e adaptadas.
