Apesar de pouco prevalente em países desenvolvidos, esta patologia continua a ter um peso significativo em certas áreas do mundo. O Centro Hospitalar e Universitário de Coimbra recebe crianças dos países africanos de língua oficial portuguesa com doença valvular de causa reumática. Nos casos de regurgitação mitral, a reparação valvular, ainda que complexa e com piores resultados em idade pediátrica, é a intervenção de escolha. O objetivo do estudo foi determinar o resultado das intervenções realizadas e procurar identificar preditores de sucesso.Métodos: Estudo observacional retrospetivo realizado em doentes com regurgitação mitral de etiologia reumática, com base em informação epidemiológica e dados de pré e pós-operatório de sintomatologia, medicação e avaliação ecocardiográfica. Foram definidos 6 critérios de sucesso com base na redução das dimensões das cavidades cardíacas esquerdas, presença de lesões residuais (regurgitação ou estenose mitral) ou de hipertensão pulmonar no pós-operatório e sintomatologia após a intervenção.Resultados: Foram estudados 30 doentes com uma média de idades de 12,53 + 3,09 anos. Cerca de 57% tinham atingimento reumático de outras válvulas, 30% tinham doença mitral mista e 60% apresentavam hipertensão pulmonar pré-operatória. Após a intervenção, 25 dos 30 doentes cumpriram pelo menos 3 dos 6 critérios de sucesso. Observou-se uma redução significativa das dimensões das cavidades cardíacas esquerdas, melhoria da gravidade da regurgitação mitral (67% dos doentes ficaram com regurgitação residual ligeira a moderada) e o gradiente transvalvular médio reduziu de 8 mmHg para 5 mmHg. Apenas um doente manteve hipertensão pulmonar e 90% apresentaram melhoria sintomática. O Z-score do ventrículo esquerdo no pré-operatório mostrou uma relação estatisticamente significativa com o sucesso (Z-score mais elevado e insucesso). A mortalidade precoce foi de 0% e a reoperação precoce de 3%.Discussão e conclusão: As intervenções realizadas tiveram, na sua grande maioria, sucesso, com grande parte dos doentes a apresentar melhoria sintomática e dos parâmetros ecocardiográficos estudados. Os resultados encontrados são concordantes com os de estudos anteriores. Estes resultados reforçam a importância do trabalho realizado e da sua continuação.Introduction: Rheumatic fever is caused by a group A streptococcal infection, that may curse with heart valve damage. Even though this is a rare disease in developed countries, it still has a big impact in other areas of the world. Centro Hospitalar e Universitário de Coimbra receives children from African countries of official Portuguese language with rheumatic valvular disease. Although complex and with worst results in children, valvular repair has shown to be the best approach in mitral valve regurgitation. The goal of this study is to determine the success of the interventions performed and to identify possible variables that may be used as predictors of that success.Methods: Retrospective observational study, performed in patients with rheumatic mitral valve regurgitation, accomplished with epidemiological information and data from before and after the interventions regarding symptoms, medication and echocardiographic evaluation. Six success criteria were defined based on the reduction of the dimensions of the left heart chambers, presence of residual lesions (mitral regurgitation or stenosis) or pulmonary hypertension post-surgery and symptoms after the interventions.Results: Thirty patients were studied with an average age of 12.53 + 3,09 years. Approximately 57% had coexisting involvement of other valves and 30% with mixed mitral lesions. 60% had pulmonary hypertension before the surgery. After the intervention, 25 of the 30 patients met 3 of the 6 criteria of success. There was a significative reduction of the dimensions of the left cardiac chambers, improvement of the severity of mitral regurgitation (67% of patients had mild to moderate residual regurgitation) and the medium transvalvular gradient reduced from 8 mmHg to 5 mmHg. Only one patient maintained pulmonary hypertension and 90% had symptomatic improvement. The left ventricle Z-score before the intervention showed a statistically significant correlation with the success (higher Z-score and worst outcome). Early mortality was 0% and early reoperation was 3%.Discussion and Conclusion: The interventions were almost all successful, with most patients presenting with improvement of their symptoms and of the echocardiographic parameters that were studied.
