Summary: Abstract Background: Expressed emotion, a qualitative measure of a family's affective attitudes and behaviors towards a relative with schizophrenia, constitutes a risk factor for relapse in these patients. The objective of this study was, by carrying out a systematic review, to understand whether higher levels of expressed emotion lead to a greater number of relapses in schizophrenic patients, leading to a worse prognosis of the disease, compared to patients living in family contexts with lower levels of this measure. Methods: A systematic review was conducted by searching articles that investigate the impact of expressed emotion on the relapse of patients with schizophrenia in PubMed, Web of Science and PsycINFO. Results: Seven articles were selected, involving approximately 1000 individuals. The criteria for diagnosing schizophrenia, the measure of expressed emotion and the definition of results were variable, but most studies found the existence of a statistically significant effect of the association investigated. Conclusion: Although there appears to be a relationship between the levels of expressed emotion and relapse in patients with schizophrenia, more population studies are needed, in different cultures and with uniform outcomes, to prove this association.
