Este subtipo molecular tem maior propensão para a metastização cerebral, por isso, requer inibidores com elevada eficácia no sistema nervoso central (SNC). Caso Clínico: Doentes do sexo masculino, diagnosticados com CPCNP estádio IV, inoperável, com rearranjo ALK positivo. Iniciaram tratamento com crizotinib, com bom controlo sistémico, porém houve progressão no SNC, levando à necessidade de substituição terapêutica. A opção foi o início de um inibidor de segunda geração, mais potente e eficaz, evitando o recurso a terapêutica local. Conclusão: O alectinib é um TKI de segunda geração que demonstrou ser eficaz na metastização intracraniana, nos doentes refratários ao crizotinib, diminuindo consideravelmente o número de metástases, permitindo adiar a utilização de radioterapia, prevenindo os seus efeitos nefastos.Introduction: The non-small cell lung carcinoma (NSCLC) with positive rearrangement of anaplastic lymphoma kinase (ALK), in advanced stage, allows a targeted therapeutic approach, through the new tyrosine kinase inhibitors (TKI). This molecular subtype is more prone to brain metastases; therefore, it requires inhibitors with high efficacy in the central nervous system (CNS). Cases Report: Male patients, diagnosed with stage IV NSCLC, inoperable, with positive ALK rearrangement. They started treatment with crizotinib, with good systemic control, but there was progression in the CNS, leading to the need for therapeutic replacement. The option was to start a second generation TKI, a more potent and effective inhibitor, avoiding the use of local therapy.
